Considerații chirurgicale diferențiate a pacienților cu ulcer perforat duodenal

Abstract

Rezumat. Au fost studiați 474 pacienți, internați de urgență cu ulcer duodenal perforat, pe perioada 1992-2020 și operati în spitalul ,,Sfânta Treime”. Incidența ulcerului duodenal perforat în această perioadă a fost de 3,8 %. Au fost studiați factorii etiopatogenetici, care afectează tabloul clinic și tratamentul chirurgical, așa ca vârsta, timpul de la debutul bolii pâna la adresarea în clinică, simptomatologia la internare, cât și tipul intervenției chirurgicale. Tipul intervenției chirurgicale a fost ales în dependență de starea generală a pacientului la internare și starea pacientului în timpul operației. Mortalitatea în lotul studiat este de 3,37%.

Summary. We studied 474 clinical observations of patients admitted urgently with perforated duodenal ulcer, for the period 1994-2020 and operated in the Sugical Clinic of the Municipal Hospital ”Holy Trinity”. The freguency of perforated duodenal ulcers during this period was 3.8%. We studied the etiopathogenetical factors, which affect the clinical picture and surgical treatment, such as age, time from onset of the disease to entering the clinic, the symptoms on admission, as well as the type of surgical intervention. Surgery was perforated according to the general condition of the patients at admission and status of each patient individually during the operation.The mortality was 3,37% in the studied lot.

Резюме Было изучено 474 клинических наблюдения пациентов, госпитализированных в срочном порядке с перфоративной. язвой двенадцатиперстной кишки, за период 1994 – 2020 годов и оперированных в хирургической клинике муниципальной клинической больницы «Святой Троицы». Частота перфоративной дуоденальной язвы в этот период составляла 3,8%. Были изучены этиопатогенетические факторы, которые влияли на клиническую картину и хирургическое лечение, такие как возраст, время от начала заболевания до поступления в клинику, симптомы при поступлении, а также вид хирургического вмешательства. Хирургические вмешательства производились в зависимости от общего состояния пациентов при поступлении и состояние каждого больного в частности во время операции. В изученном материале смертность составляет 3,37%.
