Utilizarea metodei HAL-RAR în tratamentul pacienților cu boala hemoroidală

Abstract

Rezumat. Boala hemoroidală este considerată una dintre cele mai răspîndite patologii din lume. În Republica Moldova majoritatea pacienților cu boala hemoroidală se tratează prin metode chirurgicale traumatice clasice, care sunt asociate cu sindrom dolor pronunțat postoperator, risc sporit de apariție a complicațiilor. Metode minim invazive de tratament al bolii hemoroidale reprezintă un interes deosebit. În 1995 Morinaga publică primul articol despre o nouă metodă de tratament a hemoroizilormetoda HAL (hemorrhoidal artery ligation): ligaturarea ramurilor arterei hemoroidale superioare, care ulterior în 2005 a fost perfecționată prin RAR (rectoanal repair). Scopul studiului este ameliorarea rezultatelor tratamentului chirurgical al hemoroizilor prin aplicarea metodei chirurgicale miniminvazive de ligaturare transanală a arterelor hemoroidale cu utilizarea doplerometriei ultrasonore cu mucopexie (HAL-RAR). Au fost evaluate rezultatele tratamentului chirurgical a 150 de pacienţi în anii 2014-2022 cu diagnosticul: hemoroizi cronici mixti gr. III-IV. Reieșind din datele literaturii și proprii putem constata că HAL-RAR este o metodă eficientă miniminvazivă de tratament patogenetic a hemoroizilor.

Summary. Hemorrhoidal disease is considered one of the most widespread pathologies in the world. In the Republic of Moldova, most patients with hemorrhoidal disease are treated by classic traumatic surgical methods, which are associated with pronounced postoperative pain syndrome, increased risk of complications. Minimally invasive methods of treatment of hemorrhoidal disease are of particular interest. In 1995, Morinaga published the first article about a new method of treating hemorrhoids: the HAL (hemorrhoidal artery ligation) method - the ligation of the branches of the superior hemorrhoidal artery, which later in 2005 the method was perfected by RAR (rectoanal repair). The purpose of the publication is to improve the results of the surgical treatment of hemorrhoids by applying the minimally invasive surgical method of transanal ligation of the hemorrhoidal arteries with the use of ultrasound dopplerometry with mucopexy (HAL-RAR). The results of the surgical treatment of 150 patients in the years 2014-2022 with the diagnosis: chronic mixed hemorrhoids were evaluated. III-IV. Judging from the literature and our own data, we can conclude that HAL-RAR is an effective minimally invasive method of pathogenetic treatment of hemorrhoids.

Резюме. Геморроидальная болезнь считается одной из самых распространенныхпатологий в мире. В Республике Молдова большинство больных геморроидальной болезнью лечат классическими травматичными хирургическими метода ми, которые сопряжены с выраженным послеоперационным болевым синдромом, повышенным риском осложнений. Особый интерес представляют малоинвазивные методы лечения геморроидальной болезни. В 1995 г. Моринага опубликовал первую статью о новом методе лечения геморроя: методе HAL (hemorrhoidal artery ligation) – перевязке ветвей верхней геморроидальной артерии, которая позднее в 2005 г. была усовершенствована методом RAR (rectoanal repair). Цель публикации – улучшить результаты хирургического лечения геморроя путем применения малоинвазивного хирургического метода трансанальной перевязки геморроидальных артерий с применением ультразвуковой допплерометрии с мукопексией (HAL-RAR). Были оценены результаты хирургического лечения 150 больных в 2014-2022 гг. с диагнозом: хронический комбинированный геморрой. III-IV степени. Исходя из литературных и собственных данных, можно сделать вывод, что HAL-RAR является эффективным малоинвазивным методом патогенетического лечения геморроя.
