Chisturi mucozale a sinusului maxilar. Diagnostic diferenţial și controvense în tratament

Abstract

Rezumat. Deseori clinicienii se întâlnesc în practica de rutină, cu chisturi mucozale a sinusului maxilar (CMSM). De obicei aceste chisturi se înlătură chirurgical, din motivul că medicii le confund [ cu tumori, care vor crește în dimensiuniși vor provoca complicaţii pe viitor. Scopul acestei lucrări este de a efectua reviul literaturii pentru a înţelege etiologia, patogeneza, evoluţia naturală și rezultatele la distanţa a pacienţilor operaţi prin diferite metode pentru a evita intervenţiile inutile. S-a efectuat analiza articolelor la tema respectivă disponibile pe EMBASE, Cochrane, PubMed , ScienceDirect. În studiu au fost incluse cazurile de CMSM, fără patologie concomitentă endosinusală și cu perioada îndelungată de supraveghere a pacienţilor. În total au fost analizate 87 de articole, din care au fost selectate 32, ce corespund cerinţelor sus-numite. În concluzie CMSM sunt asimptomatice, nepericuloase și de obicei nu necesită tratamentul chirugical. Metoda endoscopică se consideră gold-standard în tratamentul chirurgical a CMSM când este indicat.

Summary. Clinicians often face in their daily practice with mucosal cysts of the maxillary sinus(MCMS). Historicaly surgicaly removal was indicated, because doctors believe they have tumoral origin, grow in dimensions and complications may occur in the future. The goal of this study is to perform literature review for better understanding of etiology, pathogenesis, natural course and long-term results of patients who underwent surgical removal of MCMS using different methods to avoid unnecessary interventions. Literature review was performed using EMBASE, Cochrane, PubMed, ScienceDirect search machine. In study were included cases of MCMS situated in the maxillary sinus, without concomitant maxillary sinus pathology and with long-term followup period. 87 articles were analysed, only 32 were included in our study. Conclusion: MCMS are asymptomatic, unharmful and a great majority of them don’t need to be removed. If surgical treatment is required the endoscopic approach is gold-standart method.
