Noi ținte terapeutice în tratamentul BPOC

Abstract

Introducere. Tratamentul bronhopneumopatiei obstructive cronice (BPOC), limitat în prezent la utilizarea de bronhodilatatoare, corticosteroizi și antibiotice, necesită extinderea armamentarului terapeutic, care este strâns legată de lărgirea cunoștințelor privind patogeneza și evoluția BPOC. Scopul lucrării. Elucidarea grupelor și preparatelor noi de perspectivă în tratamentul bronhopneumopatiei obstructive cronice. Material și metode. A fost analizată baza de date Pubmed pentru a identifica grupele și preparatele de perspectivă în tratamentul BPOC, prin utilizarea cuvintelor cheie: „COPD” și „new therapeutic targets”. Rezultate. Direcțiile de cercetare a preparatelor utilizate în tratamentul BPOC s-au bazat pe mecanismele patogenetice recent identificate. Investigațiile s-au axat pe elaborarea de noi preparate din grupul GCS (ciclesonida), β2-AM (indacaterol), M-CB (glicopironiu), inhibitorilor PDE (roflumilast) și medicamentelor combinate. Concomitent, s-au evidențiat compușii noi: antagoniști cu acțiune dublă M-CB și β2-AM (batefenterol), inhibitori de kinază (imatinib), modificatori de cytokine (mepolizumab), modificatori de chemokine (plerixafor), inhibitori de NF-κB, senolitice, antioxidanți, agenți antivirali inhalatori, antifibrotici și stimulatori ai regenerării pulmonare. Concluzii. Extinderea continuă a cunoștințelor despre patogeneza și evoluția BPOC va determina identificarea de noi ținte terapeutice, potențial utile în dezvoltarea preparatelor noi utilizate în tratamentul BPOC.

Background. The treatment of chronic obstructive pulmonary disease (COPD), currently limited to the use of bronchodilators, GCS and antibiotics, requires the expansion of therapeutic armamentarium that is closely linked to the widening of knowledge on the pathogenesis and evolution of COPD. Objective of the study. Elucidation of the groups and new perspective preparations in the treatment of chronic obstructive pulmonary disease. Material and methods. The PubMed database was analyzed to identify the groups and preparations of perspective in the treatment of COPD, using the keywords „COPD” and „new therapeutic targets”. Results. Research directions for preparations used in the treatment of COPD have been based on recently identified pathogenic mechanisms. The investigations focused on the development of new preparations from the GCS group (ciclesonide), β2-AM (indacaterol), M-CB (glicopironium), PDE inhibitors (roflumilast) and combined drugs. At the same time, new compounds were highlighted: dual-action M-CB and β2-AM antagonists (batefenterol), kinase inhibitors (imatinib), cytokine modifiers (mepolizumab), chemokine modifiers (plerixafor), NF-κB inhibitors, senolytics, antioxidants, inhaled antiviral agents, antifibrotic agents and stimulators of lung regeneration. Conclusion. The continuous knowledge expansion about the pathogenesis and evolution of COPD will lead to the identification of new therapeutic targets, potentially useful in the development of new preparations utilized in the treatment of COPD.
