Aspecte etiologice și patogenetice în disfuncţia temporo-mandibulară: sinteză de literatură

Abstract

Rezumat. Introducere. Etiologia DTM rămâne și astăzi un subiect controversat în literatura de specialitate. În contextul ultimelor rezultate ale cercetării știinţifice, caracterul plurifactorial al etiologiei DTM este unanim acceptat. Unele teorii etiologice au fost abandonate, altele valorificate, venind timpul teoriilor etiologice plurifactoriale. Semnificaţia și importanţa factorilor în etiologia și patogenia disfuncţiei temporomandibulare este și astăzi un subiect de discuţie în rândul cercetătorilor și cliniciștilor, pentru stabilirea imperativelor știinţifico-didactice. Material și metode. Au fost analizate publicaţiile anilor 2000-2014, selectate pe baza unor cuvinte cheie: disfuncţie temporomandibulară, etiologia DTM, mecanisme patogenetice a diverse forme de DTM, factori etiologici, cu aplicarea unor criterii de includere și excludere. Datele obţinute, mai apoi, au fost sistematizate și analizate. Rezultate. Analizând și sintetizând datele din literatura de domeniu, referitor la etiologia și patogenia DTM, am sistematizat informaţia respectivă în funcţie de ponderea și semnificaţia factorilor condiţio-cauzali, mecanismelor de producere a disfuncţiei și interrelaţia acestora în diverse forme clinice. Concluzii. Analiza factorilor etiologici și a patogeniei, care intervin în geneza DTM, este un reper anamnestic important pentru stabilirea diagnosticului și aplicarea metodelor eficiente de tratament.

Abstract. Introduction. The etiology of TMD remains a controversial topic in the dental literature. In the context of the recent scientific research results, the plurifactorial nature of TMD etiology has been widely accepted. Some etiological theories have been abandoned, others were valorized, currently there are being proposed the plurifactorial etiological theories. The significance and importance of the factors în the etiology and pathogenesis of temporomandibular dysfunction remains even today a topic of discussion among researchers and clinicians, în order to establish scientific and educational imperatives. Material and methods. There were analyzed the articles published between 2000-2014; that were selected based on the following keywords: temporomandibular dysfunction, TMD etiology, pathogenetic mechanisms of various forms of TMD, etiologic factors. The selection process has used inclusion and exclusion criteria. The accumulated data has subjected to systematization and analysis. Results. Analyzing and synthesizing the literature of the field, on the etiology and pathogenesis of TMD, we have systematized the data depending on its weight and significance of conditional-causal factors, the mechanisms that produce dysfunction and their interrelation în various clinical forms. Conclusions. The analysis of etiological factors and pathology involved în the genesis of TMD is an important anamnestic milestone, which helps to establish the diagnosis and to select and apply effective methods of treatment.
