Introducere. Constatările imagistice ale modificărilor degenerative a coloanei vertebrale sunt prezente în proporții mari la persoanele tinere. Patogenia acestui proces degenerativ reprezintă un continuum de modificări biomecanice, care pot fi identificate in deosebi prin Imagistica prin Rezonanta Magnetica (IRM). Scop. Analiza rezultatelor obținute la IRM ce implică complexul vertebral posterior în raport cu patogenia modificărilor degenerative ale coloanei lombosacrale la tineri. Materiale și metode. Studiul s-a efectuat în baza a 100 de pacienți repartizați în 3 grupe de vârstă: 2029, 30-39 și 40-45 ani, examinați prin IRM Signa Explorer G3, 1,5 Tesla. Au fost folosite atât clasificarea radiologică a osteoartritei suprafețelor articulare (gr.0-4), cât și prevalența altor modificări degenerative, cum ar fi: clasificarea modificărilor degenerative a discului intervertebral, clasificarea Modic, stenoza de canal medular. A fost studiată literatura de specialitate relevantă. Rezultate. Prevalența degenerativă a fost mai mare în grupele de vârstă 30-39 și 40-45 ani. Osteoartroza de grad incipient a fost depistată în primul grup de vârstă, iar cea de grad avansat pentru grupul 3. Modificările după clasificare Modic se întâlnesc la 35% din pacienții examinați și sunt legate de degenerarea discului. Concluzii. Modificările degenerative ale coloanei vertebrale cresc odată cu vârsta, în special în grupul de vârstă mai avansată (40-45 ani). Multe caracteristici degenerative bazate pe IRM sunt probabil parte a îmbătrânirii și nu sunt asociate cu o simptomatică acută. Dar aceste constatări trebuie să fie interpretate în contextul stării clinice a pacientului.
Background. Imaging findings of degenerative changes in the spine are present in large proportions at young people. The pathogenesis of this degenerative process is a continuum of biomechanical changes, which can be identified in particular by Magnetic Resonance Imaging (MRI). Objective of the study. Analysis of the results obtained on MRI involving the posterior vertebral complex in relation to the pathogenesis of degenerative changes of the lumbosacral spine at young people. Materials and methods. The study was performed based on 100 patients divided into 3 age groups: 20-29, 30-39 and 40-45 years, examined by Signa Explorer G3 MRI, 1.5 Tesla. Both the radiological classification of osteoarthritis of the articular surfaces (gr.0-4) and the prevalence of other degenerative changes were used, such as the classification of degenerative changes of the intervertebral disc, the Modic classification, and the medullary canal stenosis. Relevant literature has been studied. Results. The degenerative prevalence was higher in the 30-39 and 40-45 age groups. Early-stage osteoarthritis was detected in the first age group and advanced-grade osteoarthritis for group 3. Changes after Modic score is found in 35% of patients examined and is related to disc degeneration. Conclusions. Degenerative changes in the spine increase with age, especially in the older age group (40-45 years). Many MRI-based degenerative features are probably part of aging and are not associated with acute symptoms. Nevertheless, these findings must be interpreted in the context of the patient’s clinical condition.
