Introducere. Sindromul obstructiv de apnee în somn (SOAS) este o tulburare frecventă a somnului caracterizată prin episoade recurente de colaps al căilor respiratorii superioare. Deoarece în etiologia acestor episoade se găsesc un șir de modificări la nivelul zonei orofaringiene, evaluarea locului de obstrucție este extrem de importantă. Scopul lucrării. Scopul studiului este utilizarea unei metode noi de diagnostic pentru stabilirea locului și modificărilor anatomice la pacienții cu SOAS. Materiale și metode Studiul a fost realizat pe 34 de pacienți diagnosticați cu SOAS – forma medie și severă, în clinica ORL USMF „Nicolae Testemițanu”, în perioada 2017-2022. Somnul a fost indus prin administrarea de propofol, astfel permițând examinarea pacientului în condiții similare somnului normal. Cu ajutorul endoscopul de 00 a fost posibilă vizualizarea directă a tutor structurilor anatomice. Rezultate. În urma examinării s-au pus în evidență modificări în 3 etaje ale tractului respirator superior: la nivelul foselor nazale - modificări patologice ale septului și cornetelor, zona palatală- palatul moale mai voluminos cu îngustări ale suprafeței anteroposterioare orofaringiane prin diminuarea semnificativă a distanțelor dintre peretele posterior faringian și uvulă, și zona linguală - cu modificări la nivelul bazei de limbă care, reduce spațiul cuprins între ea și peretele posterior faringian îngustând oro și hipofaringele în timpul somnului. Concluzii. În studiu au fost identificate 3 etaje ale tractului respirator superior cu modificări anatomice la pacienții cu SOAS care predispun la colapsul căilor aeriene superioare. Endoscopia în somn indus medicamentos este la moment cea mai modernă metodă de diagnostic ce ne oferă informații despre locul obstrucției și severitatea SOAS.
Background. Obstructive sleep apnea syndrome (OSAS) is a common sleep disorder characterized by recurrent episodes of upper airway collapse. Because there are a number of changes in the etiology of these episodes in the oropharyngeal area, the assessment of the site of obstruction is extremely important. Aim of study. The aim of the study is to use a new diagnostic method to determine the location and anatomical changes in patients with OSAS. Material and Methods. The study was performed on 34 patients diagnosed with OSAS - moderate and severe form, in the ENT clinic of Nicolae Testemitanu SUMPh, during 2017-2022. Sleep was induced by the administration of propofol, thus allowing the patient to be examined under conditions similar to normal sleep. With the help of the 0 0 endoscope, it was possible to directly visualize the tutor of the anatomical structures. Results. The examination revealed changes in 3 floors of the upper respiratory tract: in the nostrils - pathological changes in the septum and cornets, palatal area - softer palate with narrowing of the oropharyngeal anteroposterior surface by significantly reducing the distances between the wall posterior pharyngeal and uvula and lingual area - with changes in the base of the tongue which reduces the space between it and the posterior pharyngeal wall narrowing the oro- and hypopharynx during sleep. Conclusions. The study identified three stages of the upper respiratory tract with anatomical changes in patients with OSAS that predispose to upper airway collapse. Drug-induced sleep endoscopy is currently the most modern diagnostic method that provides us with information about the location of the obstruction and the severity of OSAS.
