Tratamentul perforațiilor posttraumatice ale timpanului

Abstract

Introducere. Perforația posttraumatică a membranei timpanice necesită o abordare terapeutică câț mai precoce în vederea restabilirii atât integrității cât și a funcției auditive. Scopul lucrării. Analiza rezultatelor tratamentului mini-invaziv la pacienții cu perforație posttraumatică a membranei timpanice. Material și Metode. Pe parcursul anilor 20202022 în clinica ORL „Emilian Coțaga”, au fost tratați 17 copii cu perforație de membrană timpanică posttraumatică. La 11 copii (65%) tratamentul a fost aplicat în primele 7 zile, perforația s-a închis prin aplicarea unui fragment de silastic pe suprafața timpanului, timp de 3 săptămâni. Alți 6 copii (35%) cu perforații mai mici, care s-au adresat mai târziu de 7 zile, au fost supravegheați și recomandate măsuri de igienă auriculară. Rezultate. La 10 copii (59%) tratați prin aplicarea fragmentului de silastic membrana timpanică a regenerat complet. La 1 copil (5%) perforația a persistat. Din cei 6 copii (35%) aflați sub supraveghere la 3 (17%) copii perforația s-a închis, iar la restul 3 copii (17%) timpanul s-a cicatrizat. Concluzii. În primele 3-7 zile de la adresarea precoce și aplicarea fragmentului biocompatibil pe membrana timpanică sporește rata de succes în perforațiile posttraumatice. În cazul perforațiilor stabile uscate peste 2 luni se recomandă de efectuat timpanoplastia.

Background. Post-traumatic perforation of the tympanic membrane requires a therapeutic approach as early as possible in order to restore the integrity and auditory function. Objective of the study. Analysis of the results of mini-invasive treatment in patients with post-traumatic perforation of the tympanic membrane Material and Methods. During the period from 2020 to 2022 year, in Emilian Coțaga ENT clinic, 17 children with post-traumatic tympanic membrane perforation were treated. In 11 children (65%) the treatment was applied in the first 7 days, the perforation was closed by applying a piece of silastic on the surface of the eardrum, for 3 weeks. Another 6 children (35%) with smaller perforations, who were presented later than 7 days, were under surveillance and recommended ear hygiene measures. Results. In 10 children (59%) treated by applying the silastic fragment, the tympanic membrane completely regenerated. In 1 child (5%) the perforation persisted. Of the 6 children (35%) under surveillance in 3 children (17%), the perforation has closed, and in the remaining 3 children (17%) the eardrum healed. Conclusion. In the first 3-7 days after the early presentation at the hospital and application of the biocompatible fragment on the tympanic membrane, it increases the success rate in post-traumatic perforations.
