Introducere. Chisturile ovariene la copii sunt rezultatul modificărilor chistice ale epiteliului tegumentar, derivați ai foliculilor și stromei ovarelor. Formațiunile chistice simple sunt foliculare și pot apărea la vârste mici sub influența estrogenică a mamei, pe când cele complexe, mari pot explica apariția torsiunii axiale în uter sau imediat în perioada postnatală. Scopul lucrării. Prezentarea cazurilor clinice după apariția unei complicații severe- ocluzie intestinală. Material și Metode. Datele anamnestice, clinice și paraclinice au fost prelevate din fișa medicală. A fost studiată literatura privind cazurile similare. Rezultate. Prezentăm două cazuri clinice la nou-născuți, în vârsta de 24 ore postnatal și alt nou-născut la 1 lună postnatal, evaluați și rezolvați chirurgical în IMSP IMșiC, CNȘP de Chirurgie Pediatrică „Natalia Gheorghiu” chirurgie neonatală, cu sindrom de durere abdominală (ocluzie intestinală). În urma examenului clinic, evolutiv, colaborate cu examenul ecografic, radiologic si TC, s-a stabilit diagnostic de formațiune de volum în cavitate abdominală. S-a intervenit chirurgical. Examenul anatomo-patologic de formațiuni chistice ovariene a stabilit diagnosticul de chisturi ovariene torsionate (torsiune necrozată în totalmente). Concluzii. Particularitatea cazurilor sunt reprezentate de cele trei procese asociate (diagnosticul de chist tumoral, inflamație acută și procesul malformativ – degenerativ), cât și de descoperirea aleatorie postnatală afecțiunii malformative, care trebuia diagnosticată antenatal.
Background. Ovarian cysts in children are the result of cystic changes in the integumentary epithelium, derived from the follicles and stroma of the ovaries. Simple cystic formations are follicular and may occur at a young age under the mother’s estrogenic influence, whereas large, complex ones may explain the appearance of axial torsion in the uterus or immediately postnatally. Objective of the study. Presentation of clinical cases after a severe complication intestinal occlusion. Material and Methods. Anamnestic, clinical and paraclinical data were taken from the medical record. The literature on similar cases was reviewed. Results. We present two clinical cases in newborns, 24 hours postnatal and another newborn at 1 month postnatal, evaluated and surgically treated in the Natalia Gheorghiu DPOA, IMC neonatal surgery, with abdominal pain syndrome (intestinal occlusion). Following the clinical examination, evolution, collaborated with ultrasound, radiological and CT, the diagnosis of volume formation in the abdominal cavity was established. Surgical intervention was performed. Pathological examination of ovarian cystic formations established the diagnosis of torsional ovarian cysts (totally necrotic torsion). Conclusion. The peculiarity of the cases is represented by the three associated processes (diagnosis of tumor cyst, acute inflammation, and malformative - degenerative process), as well as the random postnatal discovery of the given malformative condition, which was to be diagnosed antenatally.
