Managementul dilemelor morale în actul medical hepatic. Sinteza literaturii

Abstract

Introducere. Domeniul medical se confruntă zilnic cu conceptul de dilemă morală, urmărind abordarea complexă a pacientului din punct de vedere moral, profesional și legal. Datoria medicului este de a cunoaște și implementa în practică modele de soluționare a dilemelor etice. Având în vedere statutul epidemiologic și nivelul severităților patologiilor hepatice cronice, aplicabilitatea managementul dilemelor morale este mai mult ca binevenită. Scopul lucrării. Interpretarea modelelor etice privind soluționarea dilemelor morale și integrarea acestora în actul medical hepatic. Material și metode. Cercetarea constituie o sinteză a modelelor de gestionare a dilemelor etice întâlnite în practica medicală hepatică. Literatura etică și medicală a fost căutată în baze de date precum: Google Scholar, PubMed și Research4life. Preponderent s-au utilizat metodele analitică, hermeneutică și sinteza. Rezultate. În contextul analizei se constată: 1) postulatele bioetice contribuie la gestionarea situațiilor de dilemă din activitatea clinică atât a gastroenterologilor, cât și a altor specialiști; 2) domeniului medical i se propun pași bine stabiliți în gestionarea dilemelor morale, și anume: modelul tradițional, modelul moral, modelul decizional etic Murphy and Murphy. Concluzii. 1. Activitatea bioetică pledează pentru formarea statutului moral al medicului cu prezentarea postulatelor ce-i facilitează acest parcurs, specialistului îi rămâne să le aplice. 2. Gestionarea dilemelor morale în practica medicală hepatică este dependentă de condiția/ caracterul morală al medicului și implică competența și performanța acestuia de a antrena modelele etice/ bioetice în actul medical.

Background. The medical field is confronted daily by the concept of the moral dilemma, pursuing a complex approach to the patient from a moral, professional, and legal point of view. The duty of the medical doctor is to know and implement in practice models for solving ethical dilemmas. Given the epidemiological status and level of severity of liver pathologies, the applicability of moral dilemma management is more than welcome. Aim of the study. Interpretation of models of moral dilemma resolution and their integration into hepatology practice. Material and methods. The research constitutes a synthesis of models of ethical dilemma management encountered in hepatology practice. Ethical and medical literature was searched in databases such as Google Scholar, PubMed and Research4life. Predominantly analytical, hermeneutic and synthesis methods were used. Results. In the context of the analysis, it is found that: 1) bioethical postulates contribute to the management of dilemma situations in the clinical work of both gastroenterologists and other specialists; 2) the medical field is proposed well-established steps in the management of moral dilemmas, namely: the traditional model, the moral model, the Murphy and Murphy ethical decision-making model. Conclusions 1. The bioethical activity pleads for the formation of the moral status of the medical doctor with the presentation of the postulates that facilitate this path; the specialist has to apply them. 2. The management of moral dilemmas in a hepatology practice is dependent on the condition/ moral character of the doctor and involves his competence and performance to training ethical/bioethical models in the medical act.
