Introducere. Maladiile mitocondriale cauzate de mutațiile ADN-ului mitocondrial (ADNmt) sunt cel mai frecvent grup de tulburări metabolice ereditare, cauzate de defecte biochimice al lanțului respirator și al sistemului de fosforilare oxidativă. Diagnosticul acestor tulburări poate fi o provocare din cauza eterogenității clinice și a complexității genetice. Scopul lucrării. Prezentarea strategiilor actuale de diagnostic molecular-genetic a pacienților suspecți pentru tulburări mitocondriale utilizate de Laboratorul de Genetică Moleculară Umană al Institutului Mamei și Copilului. Material și metode. Prezentul studiu a fost realizat pe un grup de 43 pacienți cu fenotipul caracteristic bolilor mitocondriale. Strategia pentru diagnosticul molecular-genetic al tulburărilor mitocondriale cauzate de mutațiile ADNmt a implicat efectuarea analizei qPCR-HRM (quantitative PCR-High Resolution Melting) în scopul testării pacienților pentru cele mai frecvente mutații patogene punctiforme, urmată de secvențierea Sanger a ADNmt. Rezultate. În urma testării a 43 pacienți pentru 11 mutații punctiforme patogene comune ale ADNmt asociate cu boli mitocondriale prin tehnica qPCR-HRM, au fost identificați 3 pacienți (7%) cu mutațiile patogene m.8344 A>G, m.8993 T>G și m.11778 G>A. Conform strategiei de diagnostic molecular-genetic, în cazul în care nu au fost identificate abnormalități la nivelul ADNmt prin tehnica qPCR-HRM, pasul următor a implicat analiza spectrului de variante genetice prin secvențierea Sanger a ADNmt. Secvențierea Sanger a 21 de gene mitocondriale a fost realizată la 29 pacienți, iar în rezultat, la 12 pacienți au fost identificate mutații patogene sau potențial patogene asociate cu patologia mitocondrială (41%). Concluzii. În lotul de 43 de pacienți investigați, s-au putut identifica variante patogene și potențial patogene asociate cu fenotipul pacienților în 34% din cazuri.
Background. Mitochondrial DNA (mtDNA) diseases are the commonest group of heritable metabolic disorders, caused by a biochemical defect of the respiratory chain and oxidative phosphorylation system. Diagnosis of these disorders can be challenging due to the heterogeneity of clinical features and the complexity of the underlying genetic abnormalities. Objective of the study is to present the current molecular diagnostic strategies in patients suspected for mtDNA disorders used by the Laboratory of Human Molecular Genetics of Institute of Mother and Child. Material and methods. We analyzed mtDNA from 43 patients with common clinical features of mitochondrial disorders. The strategy for the molecular-genetic diagnosis of mitochondrial disorders caused by mtDNA mutations involved performing qPCR-HRM (quantitative PCR-High Resolution Melting) analysis in order to test patients for the most common pathogenic point mutations, followed by mtDNA Sanger sequencing. Results. After testing 43 patients for 11 common pathogenic mtDNA point mutations associated with mitochondrial diseases by the qPCR-HRM technique, 3 patients with the pathogenic mutations m.8344 A>G, m.8993 T>G and m.11778 G>A were identified (7%). Our molecular genetic diagnosis strategy provides that if no abnormalities are identified in the mtDNA analyzed by the qPCR-HRM method, the subsequent step involves the analysis of the spectrum of mtDNA genetic variants through Sanger sequencing. The Sanger sequencing technique of 21 mtDNA genes was performed in 29 patients, and as a result, pathogenic or potentially pathogenic mutations associated with mitochondrial pathology were identified in 12 patients (41%). Conclusion. In the group of 43 investigated patients, it was possible to identify pathogenic and potentially pathogenic variants associated with the patients’ phenotype in 34% of cases.
