Introducere. Despicăturile de palat sunt cele mai
frecvente malformații în cadrul malformațiilor congenitale. În funcție de clasificările existente, cele mai
multe complicații după datele bibliografice, sunt despicăturile palatiunale totale. Din cauza insuficienții de
țesut atât osos, cât și moale în partea anterioară a palatului dur, frecvent în perioada post-operatorie apar
complicații în formă de fistule oro-nazale.
Scopul lucrării: Optimizarea tratamentului chirurgical pentru diminuarea complicațiilor de origine
fistule oro-nazale anterioare la despicăturile palatinale
totale și totale transfixiante.
Metodologia. Au fost examinate 20 de dosare medicale a pacienților cu fistule oro-nazale. S-a constatat
numărul lor și cauza fistulelor oro-nazale.Rezultate. În rezultatul analizei dosarelor medicale a copiilor cu despicături ale feței, am constatat
că 40% de copii se prezintă cu despicături palatinale.
Din numărul total de despicături 15% sunt despicăturile palatinale totale și transfixiante. La toți copiii s-a
constatat insufiență de țesut osos în partea anterioară a
palatului dur. Aceste forme de despicături palatine au
prezentat rata cea mai mare de complicații a fistulelor
oro-nazale. Ceea ce reprezintă localizarea fistulelor în
regiunea anterioară a palatului dur. În tratamenul primar al despicăturilor de palat a fost propusă o modificare a metodei clasice care constă în crearea lamboului
Langhenbeck bipediculat. Metoda prevede păstrarea
continuității lamboului Langhenbeck în partea anterioară. Decolarea largă a stratului mucoperiostal se efectuează pe partea anterioară a palatului dur, în zonele
cu integritatea păstrată a țesutului osos.
Rezultate. Metoda propusă a creat condiții favorabile pentru prevenția fistulelor oro-nazale anterioare.
Toți 20 copii operați după această metodă nu au prezentat fistule oro-nazale.
Concluzie. În concluzie putem afirma că păstrarea
integrității lamboului Langhenbeck în partea anterioară a palatului dur previne formarea fistulelor oro-nazale.

palate, complications in the form of oronasal fistulas
frequently occur in the postoperative period.
Objective of the study. Optimization of surgical
treatment to reduce complications of anterior oronasal fistula origin in total and total transfixing cleft
palates.
Methodology. 20 medical files of patients with
oronasal fistulas were examined. Their number and
causative factors of oronasal fistulas were found.Results. As a result of the analysis of the medical
records of children with clefts of the face, we found
that 40% of children present with cleft palates. Of the
total clefts, 15% are total and transfixing palatal clefts.
In all children, bone tissue insufficiency was found in
the anterior part of the hard palate. These forms of cleft
palate showed the highest complication rate of oronasal fistulas. Which represents the location of fistulas in
the anterior region of the hard palate. In the primary
treatment of cleft palate, a modification of the classical
method was proposed, which consists in the creation
of a bipediculated Langhenbeck flap. The method involves preserving the continuity of the Langhenbeck
flap in the anterior part. The wide detachment of the
mucoperiosteal layer is performed on the anterior part
of the hard coat, in the areas with preserved bone tissue integrity.
Results. The proposed method created favorable
conditions for the prevention of anterior oronasal fistulas. All 20 children operated according to this method did not present oronasal fistulas.
Conclusion. In conclusion, we can state that preserving the integrity of the Langhenbeck flap in the
anterior part of the hard palate prevents the formation
of oronasal fistulas.
