Introducere. Conform rezultatelor mai multor studii, în manifestarea clinică a bolilor inflamatorii intestinale (BII) au fost descrise cazuri asociate cu cel puţin o manifestare extraintestinală. Spondiloartrita axială (SpA) a fost identificată ca fiind una din cele mai frecvente manifestări extraintestinale şi dificil de diagnosticat precoce în cadrul BII. Scopul lucrării. Evidenţierea variantelor clinice ale spondiloartritelor axiale şi caracteristicile leziunii axiale în boala inflamatorie a intestinului. Material şi metode. A fost efectuată o analiză a manifestărilor extraintestinale la un grup de pacienţi cu BII (39 pacienţi) şi cu SpA (52 pacienţi), din cadrul Spitalul Clinic Republican „Timofei Moşneaga". A fost stabilit diagnosticul de BII conform ultimilor protocoale din Republica Moldova. Diagnosticul de SpA a fost determinat în baza durerilor inflamatorii a spatelui şi modificările identificate prin metode de cercetare imagistică (RMN şi radiografia pelvină). Rezultate. După evaluarea manifestărilor extraintestinale a fost detectată artrita asimetrică la 13,19% (p < 0,05) pacienţi şi deteriorarea scheletului axial sub formă de SpA la 28,58% (p < 0,01) pacienţii cu BII. S-a observat că fiecare parametru - durerile inflamatorii de spate (r = 0,68, p < 0,05), artrită (r = 0,89, p < 0,01), artralgie (r = 0,81, p < 0,05) şi diagnosticul de boală Crohn (r = 0,77, p < 0,01) a fost asociat cu un risc crescut de diagnostic timpuriu al SpA. Concluzii. Conform rezultatelor, artrita, artralgiile şi uveita au fost descrise ca cele mai des întâlnite manifestări la pacienţii cu BII şi artropatii axiale. Iar riscul de detectare timpurie a SpA a fost stabilit ca fiind mai mare în prezenţa artritei.
Introduction. According to the results of several studies, cases associated with at least one extraintestinal manifestation have been described in the clinical manifestation of inflammatory bowel diseases (IBD). Axial spondyloarthritis (SpA) has been identified as one of the most frequent extraintestinal manifestations and difficult to diagnose early in IBD. The purpose of the work. Highlighting the clinical variants of SpA and characteristics of the axial lesion in IBD. Material and methods. An analysis of the extraintestinal manifestations was performed in a group of patients with IBD (39 patients) and with SpA (52 patients), from Timofei Moşneaga Republican Clinical Hospital. The diagnosis of IBD was established according to the latest protocols from Moldova. The diagnosis of SpA was determined based on inflammatory back pain and changes identified by imaging research methods (MRI and pelvic radiography). Results. After evaluating the extraintestinal manifestations, asymmetric arthritis was detected in 13.19% (p < 0.05) patients and damage to the axial skeleton in the form of SpA in 28.58% (p < 0.01) patients with IBD. It was observed that each parameter - inflammatory back pain (r = 0.68, p < 0.05), arthritis (r = 0.89, p < 0.01), arthralgia (r = 0.81, p < 0.05) and diagnosis of Crohn’s disease (r = 0.77, p < 0.01) was associated with an increased risk of early diagnosis of SpA. Conclusions. According to the results, arthritis, arthralgias and uveitis were described as the most common manifestations in patients with IBD and SpA. The risk of early detection of SpA was determined to be higher in the presence of arthritis.
