Introducere. Tiroidita granulomatoasă subacută (TGS) / Tiroidita De Quervain este o afecțiune inflamatorie autolim itată a glandei tiroide, caracterizată prin durere, hipertiroid ism tranzitoriu și o elevare a markerilor inflamatori. Mai multe infecții virale (gripa, COVID-19, virusul Epstein-Barr, citomegalovirus, hepatita E, virusul oreionului etc.) au fost asociate cu dezvoltarea TGS. Deși cunoscută încă din 1895, patogeneza acestei afecțiuni nu este în totalitate elucidată, însă se consideră că implică mecanisme moleculare complexe, ce provoacă alterarea integrității celulelor tiroidiene. Scopul lucrării . Elucidarea mecanismelor moleculare care stau la baza deteriorării celulelor tiroidiene în TGS, evidențiind rolul citokinelor, celulelor imune și a căilor de semnalizare apoptotice. Material și metode . Sinteză a literaturii de specialitate (47 de studii științifice și 11 studii de caz) identificate în bazele de date: PubMed, Google Scholar, Medline, WebScience și publicate în ultimii 10 ani. Rezul tate. Rezultatele indică faptul că citokinele pro-inflamatorii, precum IL-6, TNF-α și IL-1β, au un rol pivotal în medierea inflamației și periclitarea structurii tirocitelor. Tiroida este infiltrată de celule imune, inclusiv macrofage și limfocite T, exacerbând răspunsul inflamator. Căile apoptotice, în special cele implicând semnalizarea Fas/FasL, se consideră că influențează apoptoza celulelor tiroidiene. Adițional, a fost identificată o asociere dintre stresul oxidativ, disfuncția mitocondrială și afectarea celulară și inflamația în TGS. Concluzii. Înțelegerea mecanismelor moleculare ale deteri orării celulelor tiroidiene în TGS relevă ținte terapeutice po tențiale de atenuare a inflamației, oferind loc intervențiilor țintite pentru a modula activitatea citokinelor, infiltrarea celulelor imune și căilor apoptotice, cu scopul îmbunătățirii stării pacienților.

Subacute granulomatous thyroiditis (SGT) / De Quervain’s thyroiditis is a self-limiting inflammatory condition of the thyroid gland, characterized by pain, transient hyperthyroidism, and elevated inflammatory markers. Several viral infections (influenza, COVID-19, Epstein-Barr virus, cytomegalovirus, hepatitis E, mumps virus, etc.) have been associated with the development of SGT. Although known since 1895, the pathogenesis of this condition is not fully elucidated, but it is believed to involve complex molec ular mechanisms that cause thyroid cell damage. Objective of the study. To elucidate the molecular mechanisms underlying thyroid cell damage in SGT, highlighting the roles of cytokines, immune cells, and apoptotic signaling pathways. Material and methods. A synthesis of specialized literature (47 scientific studies and 11 case reports) was identified from databases such as PubMed, Google Scholar, Medline, and Web of Science, and published in the last 10 years. Results. The findings indicate that pro-inflammatory cytokines, such as IL-6, TNF-α, and IL-1β, play a crucial role in mediating inflammation and compromising the structure of thyrocytes. The thyroid gland is infiltrated by immune cells, including macrophages and T lymphocytes, exacerbating the inflammatory response. Apoptotic pathways, particularly those involving Fas/FasL signaling, are believed to influence thyroid cell apoptosis. Additionally, an association was identified between oxidative stress, mitochondrial dysfunction, cellular damage and inflammation. Conclusions . Understanding the molecular mechanisms of thyroid cell damage in SGT reveals potential therapeutic tar gets for mitigating inflammation, providing a basis for targeted interventions to modulate cytokine activity, immune cell infiltration, and apoptotic pathways to improve patient outcomes.
