Introducere. Cancerul metacron este definit ca apariția mai multor tumori maligne care se dezvoltă din diferite țesuturi cu morfologii distincte. Pacienții diagnosticați cu cancer primar au un risc de 20% mai mare de dezvoltare a unui nou cancer, ceea ce impune screeningul adecvat în această perioadă. Scopul lucrării. Prezentarea cazului clinic, al pacientului cu două tumori maligne și cu metastaze la distanță, tratat în cadrul Institutului Oncologic. Material și metode . Datele anamnestice, clinice și paraclinice au fost prelevate din fișa medicală a pacientului. Rezultate. Pacientul P., 55 ani, în 2013 diagnosticat cu cancer al colonului ascendent T2N0M0, st. I. ,pentru care suportă hemicolonectomie pe dreapta. În 2022, se adresează cu un ganglion limfatic mărit în volum, în regiunea inghinală stângă. A fost efectuată biop sia excizională a ganglionului unde se decelează metastază de melanom malign epitelioid apigmentat. Evaluat prin PETCT, care a pus în evidență multiple metastaze de melanom în ganglionii inghinali, iliaci, oase, plămâni, ficat, țesuturile moi. A urmat 9 cure de polichimioterapie, cu rezultat relativ satisfăcător, s-a obținut stabilizarea procesului neoplazic. În același timp formațiunea tumorală din lobul inferior plămân stâng a continuat să crească în dimensiuni. În ianuarie 2024 s-a efectuat lobectomie inferioară a plămânului stâng, examenul morfologic al piesei de rezecție metastază de melanom malign epitelioidocelular. Concluzii. Tratamentul personalizat și monitorizarea activă a pacientului oncologic oferă unicele posibilități reale de îmbunătățire a speranței de viață. Din acest motiv vigilența față de acest grup de pacienți trebuie să fie mai mare în comparație cu populația generală.
Background. Metachronous cancer is defined as the occurrence of multiple malignant tumors that develop from different tissues with distinct morphologies. Patients diagnosed with primary cancer have a 20% higher risk of devel oping a new cancer, which necessitates appropriate screen ing during this period. Objective of the study. To present the clinical case of a patient with two malignant tumors and distant metastases treated at the Institute of Oncology. Material and methods. The anamnestic, clinical and paraclinical data were taken from the patient’s medical record. Results. The patient P., 55 years old, was diagnosed in 2013 with ascending colon cancer T2N0M0 Stage I, for which he underwent a right hemicolectomy. In 2022, he presented with an enlarged lymph node in the left inguinal region. An excisional biopsy of the lymph node revealed metastasis of an apigmented epithelioid malignant melanoma. PET-CT evaluation highlighted multiple melanoma metastases in in guinal lymph nodes, iliac bones, lungs, liver, and soft tissues. The patient underwent nine courses of polychemotherapy, which resulted in a relatively satisfactory outcome, stabiliz ing the neoplastic process. However, the tumor in the lower lobe of the left lung continued to grow. In January 2024, a lower lobectomy of the left lung was performed, and the pathological examination confirmed metastasis of epithelioid cell malignant melanoma. Conclusion. Personalized treatment and active monitoring of oncology patients provide the only real opportunities to improve life expectancy. Therefore, vigilance towards this group of patients must be higher compared to the general population.
