Introducere. Miastenia oftalmică este cea mai frecventă formă localizată a miasteniei și afectează mușchii extraoculari, levatorul palpebral superior și mușchiul ocular orbic ular. Nefiind corect adresată din punct de vedere medical miastenia oftalmică ar putea servi ca o manifestare precoce a miasteniei generalizate. Scopul lucrării. Evaluarea con tribuției imagistice în managementul pacienților cu complicații oftalmice provocate de timom. Material și metode. Pe parcursul anilor 2014-2023 am depistat 12 pacienți cu modificări oftalmice provocate de timom: 10 pacienți au suferit de miastenie oftalmică și 2 persoane – de miastenie generalizată. Evaluarea diagnostică complexă a depistat 9 cazuri de hiperplazie timică și 3 cazuri de timom benign. 9 din 12 pacienți au fost femei, vârsta pacienților diagnosticați a variat de la 26 până la 51 de ani. Rezultate. Mani festările oftalmice ale timomului cuprind blefaroptoză, dip lopie, oboseală ușoară în caz de privire intenționată, mai rar – sindromul ochiului roșu. Radiografia orbitară poate depis ta voalarea orbitelor, tomografia computerizată – creșterea volumului și intensității semnalului din partea mușchilor perioculari. Radiografia standard a depistat lărgirea umbrei mediastinale și deformarea complexului mediastinal cu deplasarea componenților lui la nivelul etajului superior, mai ales în incidențe laterale și oblice. Tomografia computeriza tă permite o precizare definitivă a diagnosticul, diferențiind hiperplazia timică și timomul, precum și sugerând eventuală malignizare în cazuri relevante. În 9 cazuri remisie a fost realizată în urma tratamentului cu corticosteroizi, iar la 3 pacienți – după timomectomie videoscopică. Concluzii. Hiperplazia timică sau timomul se manifestă prin miastenie oftalmică sau generalizată. Evaluarea imagistică adecvată a astfel de cazuri este importantă în alegerea metodei corecte de tratament.
Background. Ophthalmic myasthenia is the most common localized form of myasthenia and affects the extraocular muscles, the upper lift of the eyelid and the orbicular ocular muscle. Not being medically properly addressed, ophthalmic myasthenia may serve as an early manifestation of generalized myasthenia. Objective of the study. Evaluation of the imaging contribution in the management of patients with thymic induced ophthalmic complications. Material and methods. During the years 2014-2023 we detected 12 patients with ophthalmic changes caused by thymic disturbances: 10 patients suffered from ophthalmic myasthenia and 2 patients – from generalized myasthenia. Complex diagnostic evaluation revealed 9 cases of thymic hyperplasia and 3 cases of benign thymoma. 9 out of 12 patients were women, the age of the patients diagnosed ranging from 26 to 51 years. Results. Ophthalmic manifestations of thymoma include blepharoptosis, diplopia, fatigue in case of intentional sighting, less often – red eye syndrome. Orbital X-ray can detect the light shading of the orbits, computerized tomography – increase in the volume and intensity of the signal from the periocular muscles. Conventional chest radiography revealed the enlargement of the mediastinum and deformation of the mediastinal complex with the displacement of its components predominantly of higher mediastinum, especially in lateral and oblique views. Computerized tomography allows definitive diagnosis, differentiat ing thymic hyperplasia and thymoma, as well as suggesting possible malignancy in relevant cases. In 9 cases remission was achieved following treatment with corticosteroids, and in 3 patients – after thymus surgery. Conclusions. Thymic hyperplasia or thymoma may manifest through ophthalmic or generalized myasthenia. Adequate imaging assessment of such cases is important in choosing the correct method of treatment.
