Introducere. Hemoragia digestivă (HD) este o urgență majoră
care pune viața în pericol. Scopul lucrării. Identificarea
elementelor cheie în asigurarea calității asistenței medicale
în managementul pacienților cu HD în DMU. Material
și metode. În cercetare au fost incluși 353 de subiecți tratați
în Departamentul de Medicină Urgentă (DMU) al IMU, pe
parcursul anului 2023, cu acuze sugestive pentru HD. Rezultate.
Din 353 cazuri, acuze la hemoragie digestivă superioară
(HDS) au fost la 305 (86,4%) pacienți, iar hemoragia digestivă
inferioară (HDI)-în 48 cazuri (13,6%). Raportul dintre
bărbați și femei a fost de 4:1. După triaj, pacienții au fost
asistați în zonele galbenă și roșie a DMU. Toți subiecții au
fost investigați clinic, paraclinic. Endoscopia digestivă superioară
(EDS) în 199 cazuri (65,24%) a infirmat diagnosticul
de HDS activă, dar a confirmat prezența leziunilor. În 106 cazuri
(34,76%) a fost confirmată hemoragia activă. Hemostaza
endoscopică (HE) prin injectarea leziunilor hemoragice
a fost efectuată în 40,57% din cazurile de HDS activă. Bandarea
endoscopică a varicelor esofagiene a fost programat
la 37 pacienți. În 26 cazuri, HE nu a fost necesară (eroziuni
hemoragice-23 cazuri, lipsa sursei de sângerare-3cazuri).
Pacienții cu HDI au fost spitalizați pentru ED. Concluzie.
Endoscopia digestivă este metoda esențială în diagnosticul
HD. Hemostaza endoscopică este metoda eficientă minim
invazivă de elecție în managementul pacienților cu HD.
Background. Gastrointestinal bleeding is a major
life-threatening emergency. Objective of the study. Identifying
the key elements in the management of patients with
digestive hemorrhage in the ED, which ensure the quality
of medical care. Material and methods. The research included
353 subjects treated in the ED of the IEM, during the
year 2023, with complaints suggestive of digestive hemorrhage.
Results. Of 353 cases of gastrointestinal bleeding,
upper gastrointestinal bleeding (UGB) was confirmed
in 305 (86,4%) cases and lower gastrointestinal bleeding
(LGB) in 48 cases (13,6%). The male to female ratio was
4:1. After triage, patients were assisted in the yellow and
red zones of the ED, depending on the values of the vital
signs parameters. All subjects were investigated clinically,
paraclinically. Upper digestive endoscopy in 199 cases
(65,24%) refuted the diagnosis of active UGB but confirmed
the presence of lesions. Active bleeding was confirmed in
106 cases (34,76%). Endoscopic hemostasis (EH) by injection
of bleeding lesions was performed in 40,57% of 106
confirmed cases of active bleeding. Endoscopic banding of
esophageal varices was scheduled in 37 patients. In 26 cases,
endoscopic hemostasis was not necessary (hemorrhagic
erosions-23cases, lack of bleeding source-3 cases). Patients
with LGB were admitted for colonoscopy. Conclusion. Digestive
endoscopy is the method of choice for the diagnosis
of gastrointestinal hemorrhage. Endoscopic hemostasis is
the effective minimally invasive method in the management
of patients with gastrointestinal bleeding.
