Introducere. Epilepsia focală farmacorezistență implică o disfuncție importantă biopsihosocială. Tratamentul chirurgical este cea mai bună opțiune pentru acești pacienți, însă doar jumătate obțin o libertate completă de crize. Această variabilitate înaltă a răspunsului postoperator implică depistarea unor factori predictivi pentru un răspuns postoper ator înalt. Scopul lucrării. Identificarea factorilor predictivi asociați unui control favorabil al crizelor epileptice postoperator (clasa Engel I). Metode și materiale. Au fost anal izate datele din ultimii 20 ani (din 2004), efectuate pe un eșantion de pacienți mai mare de 50 persoane cu epilepsie focală lezională confirmată prin IRM cerebral, cu o libertate completă de crize postoperator (clasa Engel I) și urmăriți pe o perioadă de cel puțin 1 an. Din baza de date PubMed au fost selectate articole în limba engleză și franceză. Au fost analizate 16 articole originale pe 1258 pacienți interveniți chirurgical și 2 articole de sinteză. Rezultate. Pre dictorii asociați cu un răspuns bun postoperator pe termen lung au fost: absența crizelor focale cu trecere în bilaterale tonico-clonice și o durata scurtă a epilepsiei la pacienții cu epilepsie de lob temporal cu scleroză hipocampală cărora li s-a efectuat lobectomie anterioară, iar pe termen scurt (mai puțin de 2 ani): înlăturarea obligatorie a hipocampului în epilepsia de lob temporal mezial, prezența descărcărilor epileptiforme unilaterale, rezecția completă a leziunii (de exemplu, displazie corticală) în epilepsia extratemporală. Concluzii. Este dificil de selectat un candidat ideal pentru tratament chirurgical al epilepsiei, utilizând aceleași variabile/predictori pentru toate tipurile de epilepsie. Abordarea personalizată a epilepsiei dependent de lobul implicat, etiologie, anomaliile electroencefalografie noninvazive și/ sau invazive este recomandată.
Introduction . Drug-resistant focal epilepsy involves important biopsychosocial dysfunction. Surgical treatment is the best option for these patients, but only half achieve complete seizure freedom. This high variability of postoperative response implies the detection of predictive factors for a high postoperative response. Objective of the study . Identification of predictive factors associated with a favorable outcome after surgery (Engel class I). Methods and materials . Data from the last 20 years (since 2004) were analyzed, performed on a sample of more than 50 patients with lesional focal epilepsy confirmed by brain MRI, with a complete freedom after resective surgery (Engel class I) and followed for a period for at least 1 year. Articles in English and French were selected from the PubMed database. 16 original articles on 1258 surgical patients and 2 synthesis articles were analyzed. Results. Predictors associated with a good long-term postoperative prognosis were: the absence of focal to bilateral tonic-clonic seizure and a short duration of epilepsy in patients with temporal lobe epilepsy with hippocampal sclerosis who underwent previous anterior lobectomy, and on short-term (less than 2 years): mandatory removal of the hippocampus in mesial temporal lobe epilepsy, the presence of unilateral epileptiform discharges, complete resection of the lesion (eg, focal cortical dysplasia) in extratemporal epilepsy. Conclusions. It is dif ficult to select an ideal candidate for surgical treatment of epilepsy, using the same variables/predictors for all types of epilepsy. A personalized approach to epilepsy depending on the involved lobe, etiology, noninvasive and/or invasive electroencephalographic abnormalities is recommended.
