Introducere. Hemoragiile variceale reprezintă cea mai gravă complicație a hipertensiunii portale, care în multe ca zuri devine necontrolată și duce la un sfârșit letal. Aplicarea TIPS este o metodă promițătoare care poate interveni în rezolvarea patogenetică a acestei probleme prin micșorarea hipertensiunii portale. Scopul lucrării. Studierea literaturii de specialitate și identificarea actualității metodei TIPS în raport cu alte manopere medicale și farmaceutice pentru tratamentul hemoragiilor variceale repetate. Material și metode. Metoda TIPS presupune introducerea unui cateter în vena jugulară sub control ecografic. Apoi, sub control radioscopic permanent ghidat până la nivelul ficatului, se aplică șuntul între ramurile venelor hepatice și portae. Șun tul trebuie să fie cu o acoperire specială care nu permite pro liferarea celulelor hepatice în interiorul lui, iar diametrul să nu depășească 10mm. Rezultate. Metoda TIPS s-a dovedit a fi metoda cea mai promițătoare cu rata cea mai mică de reci dive a hemoragiilor variceale. Prin micșorarea gradientului de presiune portală cu 10mmhg, s-a observat diminuarea gradului de hipertrofie al varicelor esofagiene. De asemenea, metoda TIPS mărește rata de supraviețuire a acestui grup de pacienți și reduce riscul apariției complicațiilor. Concluzii. Tratamentul definitiv al hemoragiilor variceale în contextul hipertensiunii portale ar putea fi obținut doar prin transplant hepatic, însă metoda TIPS poate fi folosită ca cea mai bună metodă paliativă, sau care temporizează cel mai mult așteptarea organului transplantat.
Background. Variceal bleeding represents the most serious complication of portal hypertension, which in many cases becomes uncontrolled and leads to a fatal outcome. The ap plication of TIPS is a promising method that can interfere in the pathogenetic resolution of this problem by reducing portal hypertension. Objective of the study. Studying the scientific literature and identifying the actuality of the TIPS method in relation to other medical and pharmaceutical procedures for the treatment of repeated variceal bleeding. Material and methods. The TIPS method involves inserting a catheter into the jugular vein under ultrasound control. The shunt is radioscopically guided to the liver and is applied between the branches of the hepatic and portal veins. The shunt must have a special coating that does not allow the proliferation of liver cells inside it, and the diameter must not exceed 10 mm. Results. The TIPS method proved to be the most promising method with the lowest recurrence rate of variceal bleeding. The degree of hypertrophy of esophageal varices was observed to decrease by reducing the portal pressure gradient to 10 mmHg. Also, the TIPS method increases the survival rate of this group of patients and reduces the risk of complications. Conclu sion. The definitive treatment of variceal bleeding on portal hypertension background can be obtained only by liver transplantation, but the TIPS method can be used as the best palliative method, or as method that delays the waiting time for liver transplantation.
