Introducere. Dereglările congenitale ale glicozilării (CDG) reprezintă o gamă largă de boli cauzate de defecte în sinteza și procesarea glicoproteinelor, cauzate de mutații în genele responsabile pentru biosinteza glicanelor. Izoelectrofocusa rea (IEF) a transferinei serice este considerată standard de aur în diagnosticul CDG. Scopul lucrării. Acest studiu are ca scop implementarea tehnicii IEF și efectuarea unui screening selectiv pentru pacienții suspectați de CDG. Material și metode. În studia s-au inclus 327 de seruri de la pacienți suspectați pentru CDG (2 luni-15 ani) cu hipotonie, convulsii, întârziere psihomotorie și implicare multisistemică cu trăsături dismorfice. IEF a transferinei a fost realizată folosind camera CSL-IEF de la Cleaver Scientific Ltd, UK, sursa de alimentare “BluePower 3000” și reactivi (geluri IEF pH 3-10 și soluții tampon) de la SERVA, Germania. Rezultate . Procesul de implementare s-a confruntat cu mai multe pro vocări, care au fost depășite prin consultări cu echipa RadboudUMC și SERVA. Până în prezent, 149 din 327 de probe de ser au fost examinate prin metoda IEF. Anomalii în glicozilarea transferinei au fost observate în 2,68% (n=4) din probele testate. Conform protocolului de diagnostic CDG, pacienții IEF pozitivi au fost testați pentru galactozemie și fructozemie. Ca urmare, un pacient a fost diagnosticat cu Galactozemie cauzată de mutației p.E203L a genei GALT, iar doi pacienți au fost diagnosticați cu Fructozemie - mutația c.113-1_115del a genei ALDOB. Pentru a identifica tipul specific de CDG la al patrulea pacient sunt necesare teste genetice suplimentare. Concluzii. Metoda IEF a fost implementată cu succes, marcând primul pas în diagnosticarea dereglările congenitale ale glicozilării la pacienții din Moldova. Acest lucru a permis identificarea a patru pacienți cu profiluri de glicozilare anormale.
Background. Congenital disorders of glycosylation (CDG) represent a family of diseases caused by errors in the synthesis, and/or processing of glycoproteins due to genetic defects involved in glycan biosynthesis. Despite various diagnostic methods for CDG, isoelectric focusing (IEF) of transferrin remains the “gold standard”. Objective of the study. to implement the IEF method and perform selective screening using IEF in patients suspected of having CDG. Material and methods. The study was conducted on 327 serum samples from CDG suspected patients (2months 15years), who presented with hypotonia, psychomotor retardation, and multisystemic involvement with dysmorphic features. IEF of transferrin was performed using the CSL-IEF chamber from Cleaver Scientific Ltd, UK, the “Blue power 3000” power source, and reagents (IEF gels pH 3-10, anode and cathode buffer) from SERVA, Germany. Results. The implementation process faced several challenges, which were overcome through consultations with the RadboudUMC team and SERVA. To date, 149 out of 327 serum samples have been screened by IEF. Abnormalities in transferrin glycosylation were observed in 2.68% (n=4) of the tested samples. According to the CDG diagnosis protocol, IEF-positive patients were tested for Galactosemia and Fructosemia. As a result, one patient was diagnosed with Galactosemia due to the p.E203L mutation in the GALT gene, and two patients were diagnosed with Fructosemia due to the c.113-1_115del mutation in the ALDOB gene. Further genet ic testing is required to identify the specific type of CDG in the fourth patient. Conclusion. The IEF method has been successfully implemented, marking the first step in the diagnosis of glycosylation disorders in patients from Moldova. This has permitted the identification of four patients with abnormal glycosylation profiles.
