Introducere. Sindromul nefrotic (SN) se caracterizează prin proteinurie masivă (>3,5 g/zi), hipoalbuminemie, edeme și hiperlipidemie. Trombembolismul este cea mai gravă complicație, datorată stării de hipercoagulabilitate, cu niveluri scăzute de antitrombină III, proteină S liberă, plasminogen și niveluri crescute a proteinelor procoagulante (fibrinogen și factorii V și VIII). Scopul lucrării. Evidențierea importanței prevenirii complicațiilor trombembolice, la copii cu SN. Material și metode. Studiul este bazat pe cercetarea lu crărilor științifice din bazele PubMed, UpToDate, Cochrane Library, publicate în aa. 2014-2024 . Rezultate. Trombembolismul arterial și venos este asociată cu SN. Boala trombo embolică venoasă (TEV) este o complicație rară, dar adesea devastatoare, a SN la copii. Doar 3% dintre copiii cu SN dez voltă TEV semnificativ clinic, iar alți 24-25% sunt afectați de TEV subclinic. Gradul de hipoalbuminemie și proteinurie sunt factori de risc substanțiali pentru evenimente trombembolice. Ghidul KDIGO recomandă ca profilaxia cu anticoagulante să fie luată în considerare pentru pacienții cu SN care prezintă niveluri de albumină serică <20–25 g/litru. Simptomele frecvente asociate cu TEV acut la copii includ: extremități edemațiate, dureroase, sindromul venei cave su perioare. La copiii cu SN și trombembolism, timpul median până la prezentarea primului eveniment tromboembolic a constituit 70,5 zile și 61% dintre evenimentele trombembolice s-au prezentat în primele 3 luni de la diagnosticul SN. Ghidul KDIGO recomandă ca terapia anticoagulantă profilactică la pacienții cu SN să se bazeze pe o estimare a riscului de evenimente trombotice și a riscului de complicații hemoragice. Concluzii. Recunoașterea pacienților cu risc crescut și inițierea unei profilaxii adecvate este impera tivă pentru a preveni evenimentele și complicațiile asociate. Tratamentul TEV se bazeză pe comorbiditățile pacientului și pe factorii de risc pentru complicațiile asociate cu anumite anticoagulante.

Thromboembolism is the most serious complication, due to the hypercoagulable state, with low levels of antithrombin III, free protein S, plasminogen, and increased levels of procoagulant proteins (fibrinogen and factors V and VIII). Objective of the study . Highlighting the importance of preventing thromboembolic complications in children with NS. Material and methods. The study is based on the research of scientific works from PubMed, Up ToDate, Cochrane Library databases, published in aa. 20142024. Results . Arterial and venous thromboembolism is as sociated with SN. Venous thromboembolic disease (VTE) is a rare but often devastating complication of NS in childhood. Only 3% of children with NS develop clinically significant VTE, and another 24-25% are affected by subclinical VTE. The degree of hypoalbuminemia and proteinuria are substantial risk factors for thromboembolic events. The KDIGO guideline recommends that prophylactic anticoagulation be considered for patients with NS who have serum albumin levels <20–25 g/liter. Common symptoms associated with acute VTE in children include a swollen, painful extremity that is sometimes plethoric, superior vena cava syndrome. In children with NS and thromboembolism, the median time to the first thromboembolic event was 70.5 days, and 61% of thromboembolic events occurred within the first 3 months of SN diagnosis. The KDIGO guideline recommends that prophylactic anticoagulant therapy in patients with NS should be based on an estimate of the risk of thrombotic events and the risk of bleeding complications. Conclusion. Recognition of high-risk patients and initiation of appropri ate prophylaxis is imperative to prevent such events and as sociated complications. Treatment of VTE should be based on the patient’s comorbidities and risk factors for complications associated with certain anticoagulants.
