Introducere. Sindromul Alport (SA) este o afecțiune genet ică rară, care evoluează spre Boala Cronică de Rinichi (BCR). Conform rapoartelor U.S. Renal Data System (USRDS), 3% dintre toți copiii cu BCR în stadiu terminal sunt diagnosticați cu SA. Scopul lucrării. Prezentarea unui caz clinic de progresie a SA spre BCR. Material și metode. Pacient de 9 ani, sex masculin, internat în secția de Nefrologie a IMC, cu acuze la micțiuni rare, cantitativ reduse, sindrom edematos, dureri abdominale. Din istoricul bolii: copilul diagnos ticat cu surditate neurosenzorială bilaterală la vârsta de 3 ani. Din anamneza eredocolaterală: tatăl prezintă surditate, efectuează hemodializă cronică. Rezultate. Starea generală a copilului - gravă, determinată de sindromul edematos și retenție azotată. Paliditatea tegumentelor, edeme periorbit ale, TA-120/95 mm/Hg, retard staturo-ponderal: G-27 kg (9 p., -1,35 z), talia - 129 cm (3 p., -1,83 z); IMC-16,2 (38 p, -0,31 z). Paraclinic: Hemoleucograma relevă Hb-106 g/l; Ex amenul biochimic al sângelui evidențiază sindromul de retenție azotată (creatinina – 252 μmol/l, ureea – 9,8 mmol/l, eRFG – 17 ml/min/1,73 m 2 ), hiperkaliemie- K-5,62 mmol/l; EAB-acidoză metabolică. Sumarul urinei atestă proteinurie – 0,22 g/l, leucociturie până la 8-10 c/v, hematurie 14-15 c/v. Proba Niciporenko - Leucocite -1,0 x10 6 /L, Eritrocite 8,0 x 10 6 /L. Proteinuria timp de 24 h – 0,32 g. La examenul ecografic al rinichilor se vizualizează - RD= 45× 26 mm, RS=96 × 42 mm, ecogenitate sporită, contur șters; Rezultatele molecular-genetice (prin secvențiere de generație ur mătoare (NGS) cu paneluri țintă): anomalii în gena COL4A5. Luând în considerare acuzele la internare, istoricul bolii și examenul clinico-paraclinic, testele genetice, se stabilește diagnosticul clinic: Sindromul Alport. Boală cronică de rinichi gr.IV. Nefroscleroză bilaterală. Anemie gr.I. Tratament medicamentos cu nefroprotectoare, corecția dereglărilor metabolice, electrolitice, dezagregante, preparate de fier. Concluzie. Detectarea precoce a SA permite inițierea tratamentului, reducerea evoluției nefavorabile a procesului renal cu instalarea progresivă a BCR și îmbunătățirea prog nosticului pe termen lung.
Background. Alport syndrome (AS) is a rare genetic disorder that can lead to chronic kidney disease (CKD). According to U.S. reports Renal Data System (USRDS), 3% of all children with end-stage CKD are diagnosed with AS. Objec tive of the study. Presentation of a clinical case of AS progression to CKD. Material and methods. A 9-year-old patient boy was hospitalized in the Nephrology department of IM and C, with complaints of infrequent urination, reduced quantity, edematous syndrome, and abdominal pain. From medical history: the child was diagnosed with bilateral sen sorineural deafness at the age of 3. From the family history: the father has deafness and performs chronic hemodialysis. Results. Objective: serious general condition, determined by edematous syndrome and azotemia. Pallor of the skin, periorbital edema, BP-120/95 mm/Hg, weight retardation: G-27 kg (9 p., -1.35 z), waist-129 cm (3 p, -1.83 z); BMI-16.2 (38p, -0.31z). Paraclinical: Hb-106 g/l; The biochemical examination of the blood reveals nitrogen retention syndrome (creatinine – 252 μmol/l, urea – 9.8 mmol/l, eRFG – 17 ml/ min/1.73 m2), hyperkalemia- K-5.62 mmol/l; EAB-metabolic acidosis. The urine summary shows proteinuria - 0.22 g/l, leukocyturia up to 8-10 c/v, hematuria 14-15 c/v. Niciporenko test - Leukocytes -1.0 x10 6 /L, Erythrocytes 8.0 x 106 /L. Proteinuria during 24 h – 0.32 g. The ultrasound examination of the kidneys showed - RD= 45 × 26 mm, RS=96 × 42 mm, increased echogenicity; Molecular genetic results (next-generation sequencing (NGS) with target panels): abnormalities in the COL4A5 gene. Considering patients’ complaints, the history of the disease, the clinical-paraclinical examination and genetic tests, the clinical diagnosis is established: Alport Syndrome. Chronic kidney disease gr.IV. Bilateral nephrosclerosis. Gr. I Anemia. The treatment was with nephroprotectors, correction of metabolic, electrolytic, disaggregating disorders, and iron preparations. Conclu sion. Early detection of AS allows the initiation of treatment, reduction of the unfavorable evolution of the renal process with the progressive installation of CKD, and improvement of the long-term prognosis.
