Introducere. Analiza profilului lipidic la femeile de vârstă reproductivă după menopauza chirurgicală prezintă un interes major pentru medicii ginecologi deoarece histerectomia rămâne în continuare una din cele mai răspândite intervenții ginecologice și reprezintă tratamentul irevocabil a patologiilor ginecologice benigne. Scopul lucrării. Scopul studiului a fost de a analiza profilul lipidic la femeile de vârstă reproductivă după menopauza chirurgicală. Material și metode. A fost realizat un studiu prospectiv care a inclus 30 de cazuri de histerectomie fără ovarectomie (lotul I) și 30 de cazuri de histerectomie cu ovarectomie bilaterală (lotul II). Lotul de control a fost constituit din 30 femei de vârstă similară. Nivelul seric al colesterolului total, trigliceridelor, HDL-C și LDL-C a fost evaluat preoperator, la a 10-12-a zi postoperator, la 6 și la 12 luni după intervenție. Rezultate. Analiza profilului lipidic a evidențiat o creștere semnificativă a nivelului de colesterol total de la 3,91 ±0,37 mmol/l preoperator la 4,18 ±0,43 mmol/l la 12 luni postoperator în lotul I și de la 4,11 ±0,32 mmol/l la 4,28 ±0,21 mmol/l în lotul II. Aceste valori au fost semnificativ mai mari în comparație cu lotul de control (3,92 ±0,59). De asemenea rezultatele studiului relevă faptul că nivelul seric de trigliceride la 12 luni postoperator a fost semnificativ mai mare în ambele loturi (1,85 ±0,25 mmol/l în lotul I și 1,82 ±0,16 mmol/l în lotul II) în comparație cu lotul de control (1,63 ±0,17 mmol/l). Creșterea nivelului seric de LDL-C pre cum și scăderea HDL-C a fost mai evidentă la subiecții din lotul II. Concluzii. Histerectomia cu sau fără ovarectomie a condus la dereglări ale profilului lipidic în primul an postoperator. Diagnosticul și tratamentul individualizat precoce pot preveni declinul hormonal, apariția complicațiilor și menține calitatea vieții la această categorie de paciente.
Background. The analysis of the lipid profile in women of reproductive age after surgical menopause is of great inter est to gynecologists because hysterectomy remains one of the most common types of gynecologic interventions and represents the treatment of definitive for benign pathologies. The aim of the study. The aim of the study was to analyze the lipid profile in women of reproductive age after surgical menopause. Material and methods. A prospective study was carried out that included 30 cases of hysterectomy without ovariectomy (group I) and 30 cases of hysterectomy with bilateral ovariectomy (group II). The control group consisted of 30 women of similar age. Serum level of total cholesterol, triglycerides, HDL-C and LDL-C were evaluated preoperatively, on the 10th-12th postoperative day, 6 and 12 months after the intervention. Results. The lipid profile analysis showed a significant increase in the mean total cholesterol level from 3.91 ±0.37 mmol/l preoperatively to 4.18 ±0.43 mmol/l 12 months postoperatively in group I and from 4.11 ±0.32 mmol/l to 4.28 ±0.21 mmol/l in group II. These values were significantly higher compared to the control group (3.92 ±0.59). Also, the results of our study reveal that the serum level of triglycerides at 12 months postoperatively was significantly higher in both groups (1.85 ±0.25 mmol/l in group I and 1.82 ±0.16 mmol/l in group II) compared to the control group (1.63 ±0.17 mmol/l). The increase in serum LDL-C level as well as the decrease in HDL-C was more evident in the subjects of group II. Conclusions. Hysterectomy with or without ovariectomy led to disturbances in the lipid profile in the first postoperative year. Early diagnosis and individualized treatment can prevent hormonal decline, the appearance of complications and can also maintain the quality of life in this category of patients.
