Introducere. Hemangiomul reprezintă o tumoare benignă formată dintr-o aglomerare anormală de vase de sânge, afectând zona oromaxilofacială și influențând funcționalitatea și estetica. Managementul chirurgical necesită înțelegerea factorilor etiologici, a tabloului clinic și a tehnicilor de diagnostic pentru un tratament eficient. Scopul lucrării. Prezentarea unui caz complex de hemangiom oromaxilofacial și abordarea chirurgicală aplicată, evidențiind factorii etiologici, metodele de evaluare clinică și tehnicile de diagnostic utilizate. Material și metode. Pacienta C.A./62 ani s-a prezentat din motiv a dereglării esteticii, fără durere și disfuncție masticatorie, cu o formațiune tumorală în regiunea buzei inferioare. S-a examinat clinic și s-a solicitat examinarea histologică, stabilind diagnosticul de Hemangiom cavernos în regiunea buzei inferioare pe linia mediană. Re zultate. Tratament: Sub anestezie loco-regională cu soluție de articaină 1 ml – 4%,s-a efectuat incizia în felie de portocală în limitele terenului sănătos, formațiunea fiind excizată în totalitate în limitele stratului muscular și efectuată ligatu rarea cu fir rezorbabil a două vase care alimentau formațiunea, obținând hemostaza necesară. Prelucrarea antiseptică. Suturarea stratificata. S-a efectuat pansamentul a 2-a zi, administrarea tratamentului medicamentos, recomandări postoperatorii. Hemangioamele reprezintă o varietate de forme morfopatologice, cu o diversă localizare, dimensiuni și profunzime. Fiecare caz clinic necesită o abordare individuală corectă, cât pentru diagnostic, atât și pentru tratament. Concluzii. Hemangioamele oro-maxilo-faciale, tumori vasculare benigne, pot provoca un impact major asupra funcționalității și esteticii. Managementul chirurgical, bazat pe înțelegerea etiologiei și tehnicilor avansate de diagnostic, a permis un tratament eficient, cu recuperare rapidă și fără complicații majore.
Background. A hemangioma is a benign tumor formed by an abnormal cluster of blood vessels, affecting the oromaxillofacial area and influencing both functionality and aesthetics. Surgical management requires an understanding of the etiological factors, the clinical picture, and diagnostic techniques for effective treatment. Objective of the study. To present a complex case of oromaxillofacial hemangioma and the applied surgical approach, highlighting the etiological factors, clinical evaluation methods, and diagnostic techniques used. Material and methods: Patient C.A., a 62-year-old female, presented due to aesthetic concerns, without pain or masticatory dysfunction, with a tumor formation in the region of the lower lip. Clinical examination and histological assessment were performed, establishing the diagnosis: hemangioma in the lower lip region on the midline. Results. Treatment: Under loco-regional anesthesia with 1 ml of 4% articaine solution, an incision in the shape of an orange slice was made within the healthy tissue boundaries. The formation was completely excised within the muscle layer, and two vessels feeding the formation were ligated with absorbable sutures, achieving the necessary hemostasis. Antiseptic processing and layered suturing were performed. A dressing was applied on the second day, along with administration of medication and postoperative recommendations. Hemangiomas exhibit a variety of morpho-pathological forms with diverse locations, sizes, and depths. Each clinical case requires a correct individual approach for both diagnosis and treatment. Conclusions. Oromaxillofacial hemangiomas, benign vascular tumors, can significantly impact functionality and aesthetics. Surgical management, based on understanding the etiology and advanced diagnostic techniques, allowed for effective treat ment, rapid recovery, and no major complications.
