Introducere. Dereglările temporomandibulare (DTM) au prevalență maximă la persoanele de vârstă medie (35-50 ani), însă în ultimele decenii, sporește rata de afectare a persoanelor tinere, nefiind pe deplin investigate particular itățile de manifestare a bolii în acest grup de vârstă. Scopul lucrării. Investigarea expresiei sindromului algic DTM la adolescenți și tineri adulți. Material și metode. Între 20222023, la bazele catedrelor de Stomatologie Terapeutică/ OMF pediatrică și pedodonție “Ion Lupan” au fost investigați 37 de tineri cu DTM miogen-artrogene (în baza DC/TMD) – 19 adolescenți (10 fete, 9 băieți); 18 tineri adulți (14 femei, 4 bărbați). S-au evaluat situsurile dureroase stomatognatice (număr, locație, algometrie). Rezultate. Diferențe privind distribuția per sexe s-au observat doar la tinerii adulți, predominând femeile (Chi 2 = 5.65, p = 0.0174). Adolescenții prezentau mai multe situsuri stomatognatice dureroase față de adulți tineri (13.72±3.1 vs. 10.34±1.21), cu o ponde re mai mare de afectare a regiunilor musculare față de cele articulare. Pragurile de sensibilitate algică la presiune erau mai reduse la sexul feminin comparativ cu cel masculin, atât la nivelul mușchilor maseteri (0.64±0.12 vs. 0.89±0.7), cât și articular (0.34±0.2 vs. 0.43±0.4). Concluzii. La tinerii cu DTM, sindromul algic se manifestă mai extins și intens, cu un număr mai mare de situsuri dureroase și praguri de sensibilitate mai reduse, în special la femei. Aceste constatări sugerează necesitatea unor abordări personalizate în managementul DTM la tineri, ținând cont de diferențele de vârstă și sex.
Background. Temporomandibular disorders (TMD) have a maximum prevalence in middle-aged people (35-50 years old), but in recent decades, the rate of among young people has increased, while the particularities of the manifestation of the disease in this age group have not been fully investigated. Objective of the study. Investigating the expression of TMD pain syndrome in adolescents and young adults. Material and methods. Between 2022-2023, 37 young people with myogenic-arthrogenic TMD (based on DC/TMD) were investigated at the bases of the Departments of Therapeutic Dentistry/Pediatric OMF and Pedodontics “Ion Lupan” - 19 teenagers (10 girls, 9 boys); 18 young adults (14 women, 4 men). Painful stomatognathic sites (number, location, algometry) were evaluated. Results. Differences in gender distribution were observed only in young adults, women predominating (Chi 2 = 5.65, p = 0.0174). Adolescents presented more painful stomatognathic sites compared to young adults (13.72±3.1 vs. 10.34±1.21), with a greater share of damage to the muscular regions compared to the articular ones. Thresholds of pain sensitivity to pressure were lower in females compared to males, both at the level of masseter muscles (0.64±0.12 vs. 0.89±0.7) and articular site (0.34±0.2 vs. 0.43±0.4). Conclusion. In young people with TMD, the pain syndrome is more extensive and intense, with a greater number of painful sites and lower sensitivity thresholds, especially in women. These findings suggest the need for tailored approaches in the management of TMD in young people, taking into account age and gender differenc es.
