Introducere. Recidiva tuberculozei reprezintă un nou
episod al bolii, dezvoltat după vindecare sau după încheierea
celei mai recente cure de tratament. Spectrul rezistenţei
medicamentoase permite diferenţierea reinfecţiei exogene de
reactivarea infecţiei tuberculoase latente. Scopul studiului a constat în evaluarea caracteristicilor sociale, demografice şi
economice, precum şi identificarea factorilor de risc ai pacienţilor
cu recidivă a tuberculozei pulmonare.
Material şi metode. A fost realizat un studiu retrospectiv,
comparativ, care a inclus 119 pacienţi cu recidivă a tuberculozei,
diagnosticaţi în perioada anului 2015. Aceştea au fost distribuiţi
în 2 loturi: cel cu TB sensibilă (n=57) şi cel cu TB-MDR
(n=62). Comparaţi parametrii socio-demografici şi de abordare
a cazului. Statistică descriptivă.
Rezultate. Recidiva tuberculozei pulmonare are o pondere
importantă în indicatorul incidenţei globale şi demonstrează
persistenţa factorilor de risc de îmbolnăvire. Pentru TB-MDR
a fost caracteristic: vârsta tânără, reşedinţa urbană şi statutul
social-economic defavorizat, iar pentru TB sensibilă – vârsta
înaintată şi reşedinţă rurală. Examinarea simptomaticilor a
permis identificarea cazurilor cu TB sensibilă, iar transferul
din alte instituţii medico-sanitare a permis confirmarea diagnosticului
de recidivă a TB-MDR. Cea mai mare rată a pacienţilor
cu TB sensibilă au fost trataţi cu succes şi o proporţie
similară de cazuri din ambele eşantioane au decedat pe parcursul
tratamentului.
Concluzie. Particularităţile socio-demografice şi de management
al pacienţilor contribuie la dezvoltarea recidivei cu diferit
spectru de rezistenţă medicamentoasă a micobacteriilor.
Introduction. Tuberculosis relapse represents a new episode,
developed after healing or treatment completion of the
latest treatment cure. The drug resistant spectrum permits
the differentiation of exogenous infection from the reactivation
of latent tuberculosis infection. The research aim was the assessment of social, demographic and economic characteristics,
as well as identification of the risk factors of patients
with relapse of pulmonary tuberculosis according to the drug
resistance spectrum.
Material and methods. A retrospective, cohort, descriptive
study which included 119 patients with tuberculosis relapse
diagnosed during 2015, distributed in a drug susceptible
TB group – 57 patients and a second group of 62 patients with
MDR-TB was performed. Were compared social, demographic
and case management parameters. Descriptive statistics.
Results. Pulmonary tuberculosis relapse constitutes a high
proportion within the global incidence value and demonstrates
the maintenance of the risk factors for the morbidity. For MDRTB
patients were characteristic young age, urban residence
and social economic vulnerable state, but the susceptible TB
old age and rural residence. The examination of the symptomatic
cases permitted the identification of the drug susceptible
TB cases, but the transfer from other medical-sanitary institution
permitted the diagnosis of the MDR-TB relapse. The highest
rate of patients with drug susceptible TB was successfully
treated and a similar proportion died during the treatment.
Conclusion. Social demographic features and management
of patients contributed to the development of the relapse with
different spectrums of the drug-resistant Mycobacterium.
