Introducere. Infecţia ocultă cu virusul hepatitei B (IOB) rămâne o entitate subdiagnosticată în pediatrie, cu implicaţii importante asupra evoluţiei şi prognosticului afectării hepatice cornice, impunând elaborarea unor protocoale de screening mai eficiente şi supravegherea continuă a pacienţilor pediatrici. Scop. Scopul acestui studiu a fost de a determina ponderea infecţiei oculte cu virusul hepatitei B la pacienţi care manifestă semne clinice de hepatomegalie şi modificări patologice ale probelor hepatice. Material şi metode. Studiul a inclus 216 copii (1-17 ani, media: 7,6 ± 2,5 ani), internaţi pentru hepatomegalie şi citoliză hepatică. Loturile au inclus: IOB (confirmată serologic şi molecular), HVBC (confirmată serologic şi cu ADN-VHB detectabil) şi hepatite de altă etiologie. Datele au fost obţinute din fişele medicale ale pacienţilor în perioada 2018-2025. Rezultate. Din numărul total de 216 pacienţi, 15,3% (n = 33) au fost diagnosticaţi cu IOB, 16,2% (n = 35) cu HVBC, iar 68,5% (n = 148) cu hepatite de alte etiologii. Dintre aceştia, 40,8% au prezentat afecţiuni infecţioase non-VHB (infecţie cu virus Epstein-Barr, CMV, VHC), cele mai frecvente fiind mono-nucleoza infecţioasă (19,0%) şi infecţiile cu CMV sau VHC (câte 10,2%). Restul de 58,6% au avut afecţiuni neinfecţioa-se: hepatite toxice (23,1%), hepatite medicamentoase (6,8%) şi tulburări metabolice, cum ar fi sindromul Gilbert (6,1%), alături de alte cauze rare (autoimune, genetice sau idiopatice), fiecare având o prevalenţă sub 5%. Concluzii. Ponderea înaltă, comparabilă, a IOB cu HVBC diagnosticate susţine necesitatea screeningului imunoserologic pentru infecţia ocultă cu VHB la copii. Rezultatele subliniază riscul de subdiagnosticare în absenţa testării pentru anti-HBcore sumar şi ADN-VHB la pacienţii cu semne sugestive de hepatită.

Occult hepatitis B virus infection (OBI) remains an underdiagnosed entity in pediatrics, with significant implications for the progression and prognosis of chronic liver disease, requiring the development of more effective screening protocols and continuous monitoring of pediatric patients. Objective. The aim of this study was to assess the prevalence of occult hepatitis B virus infection in children with clinical signs of hepatomegaly and abnormal liver function test results. Material and methods. The study included 216 children (1-17 years, mean age 7.6 ± 2.5) hospitalized for hepatomegaly and hepatic cytolysis. The groups included: OBI (serologic and molecular confirmation), CHB (confirmed by complete serology and detectable HBV DNA), and other hepatitis. Data were obtained from patients’ medical records between 2018 and 2025. Results. Out of a total of 216 patients, 15.3% (n = 33) were diagnosed with OBI, 16.2% (n = 35) with CHB, and 68.5% (n = 148) with hepatitis of other etiologies. Among these, 40.8% had non-HBV infectious conditions (Epstein-Barr virus infection, CMV infection, HCV infection), the most frequent being infectious mononucleosis (19.0%) and CMV or HCV infections (each 10.2%). The remaining 58.6% had non-infectious liver diseases: toxic hepatitis (23.1%), drug-induced hepatitis (6.8%), and metabolic disorders such as Gilbert syndrome (6.1%), along with other rare causes (autoimmune, genetic, or idiopathic), each with a prevalence below 5%. Conclusion. The high and comparable prevalence of OBI and CHB highlights the need for comprehensive immunoserologic screening for occult HBV infection in children. The findings underscore the risk of underdiagnosis in the absence of an-ti-HBc total and HBV DNA testing in pediatric patients.
