Încărcarea imediată a implanturilor dentare endoosoase şi avantajele restaurării temporare

Abstract

Introducere. Restaurarile cu suport implantar pentru pa-cientii parţial edentaţi sunt o modalitate de tratament previzibilă. Coroanele provizorii pot fi utilizate după o perioadă adecvată de vindecare. Implantele dentare au o contribuţie majoră în calitatea vieţii ce restaurează funcţiile sistemului stomatognat. Scop. Analiza particularităţilor tratamentului implanto-protetic cu încărcare imediată cu coroane acrilice pe suport implantar pentru refacerea integrităţii arcadelor dentare ale sistemului stomatognat. Material şi metode. Din 2021-2025, 30 pacienţi: 15 femei şi 15 bărbaţi, vârsta 25 - 55 de ani s-au adresat la Clinica Stomatologică Universitară Nr. 2, pentru inserarea a 60 IDE pentru tratamentul EPF cu CT care sunt apoi înlocuite cu CD. Protocolul ÎI a IDE a inclus valorile Periotest cu indicii -4 şi -7, SPIDE - 40-45 Ncm, şi revers torgue - 30-35 Ncm. Rezultate. ÎI a IDE în zona estetică este un concept clinic viabil şi duce la rezultate favorabile ale tratamentului din punct de vedere estetic. Pe lîngă efectele benefice ale încărcării imediate a IDE induce osteogeneza şi o stabilitate primară şi secundară a IDE. Grosimea gingiei a fost - 5.93 ± 0,24 mm în grupul de bărbaţi şi 3.67 ± 0,25 mm în grupul de femei (p> 0.05). După analiza statistică au fost detectată o corelaţie negativă între grosimea gingiei şi recesiunea gingivală. Valorile Periotest au fost -3.9 ± 0.32 (b.) şi -4.6 ± 0.16 (f.) (p> 0.05). Mann Whitney test şi t-Student test nu au arătat nici o diferenţă statistică între grupuri. Concluzii. Restaurările provizorii utilizate pentru evaluarea funcţiei estetice şi fonetice înainte de confecţionarea restaurărilor implanto-protetice definitive, stopînd migrările dentare în plan vertical şi orizontal, păstrând starea ţesuturilor periimplantare şi gingivale şi restaurarea sistemului dentar.

Dental implants have a major contribution in quality of life by restoring the affected functions of the stomatognathic system. Objective. Analyzing the features of the implant-prosthetic treatment with immediate temporary crowns loading for restoring the integrity of the dental arches of stomatognathic system Material and methods. From the years 2021-2025, 30 patients:15 females and 15 males, age 25 to 55 years presented to the University Stomatological Clinic Nr.2 for placement 60 DI and restore FEA with AC after change with PC. Protocol with IL of DI included the Periotest values - 4 and -7, PSDI - 40-45 Ncm, and the reverse torque of 30-35 Ncm Results. The IL of DI in esthetic zone is a viable clinical concept and lead to favorable treatment outcomes. Besides the beneficial effects of IL of DI might induce osteogenesis and good stability of the DI. All DI were successfully osseointegrated. The gingiva thickness - 5.93 ± 0,24 mm in the man group and 3.67 ± 0,25 mm in women group (p> 0.05). After the statistical analysis it was revealed a negative correlation between the gingiva thickness and mucosal recession. The average Periotest values -3.9 ± 0.32 (men), and -4.6 ± 0.16 (women) (p> 0.05). Mann Whitney test and t-Student test showed no statistical difference between groups. Conclusion. Provisional restorations used to evaluate aesthetic and phonetic function prior to delivery of the final implant restorations, stopping dental migrations in the vertical and horizontal plane, while preserving the condition of the peri-implant and gingival tissues and restore dental system
