Rezumat Fibroza reprezintă stadiul final al proceselor inflamatorii cronice, contribuind la apariția unor disfuncții organice severe. Miofibroblastele, activate prin mecanisme complexe (TGF-β/SMAD, Wnt/β-catenină), joacă un rol central în progresia fibrozei, atât la nivel renal, cât și hepatic. În plus, tranziția epitelial-mezenchimală (EMT) și expresia crescută a Galectinei-3 amplifică răspunsurile profibrotice. A fost realizat un studiu bibliografic pe baza articolelor indexate în PubMed privind fibroza renală și hepatică. Activarea miofibroblastelor, provenite din multiple surse (fibroblaste rezidente, tranziție epitelial-mezenchimală – EMT, celule stelate hepatice – HSC), conduce la acumularea excesivă de matrice extracelulară (ECM) și la progresia fibrozei. TGF-β1 și calea Wnt/β-catenină joacă un rol esențial în reglarea acestor procese, iar galectina-3 se evidențiază atât ca biomarker, cât și ca potențială țintă terapeutică. La nivel renal, TGF-β1 activează Smad2/3, stimulând expresia colagenului și a α-SMA. În ficat, activarea HSC și transformarea acestora în miofibroblaste sunt strict controlate de TGF-β și de interacțiunea cu calea Wnt. Conexiunile funcționale și moleculare dintre ficat și rinichi sugerează că fibrogeneza unui organ poate influența progresia fibrozei în celălalt. Înțelegerea mecanismelor comune și interconectate ale fibrogenezei hepatice și renale deschide perspective pentru dezvoltarea unor strategii terapeutice inovatoare. Inhibarea căilor TGF-β/SMAD, Wnt/β-catenină și a galectinei-3 ar putea oferi un efect antifibrotic dual, cu potențial de a limita simultan progresia fibrozei hepatice și renale.
Summary
Fibrosis represents the final stage of chronic inflammatory processes, contributing to the development of severe organ dysfunction. Myofibroblasts, activated through complex mechanisms (TGF-β/SMAD, Wnt/β-catenin), play a central role in the progression of fibrosis, both in the kidney and the liver. Additionally, epithelial-mesenchymal transition (EMT) and increased expression of Galectin-3 amplify profibrotic re-sponses. A bibliographic study was conducted based on articles indexed in PubMed concerning renal and hepatic fibrosis.Activation of myofibroblasts, derived from multiple sources (resident fibroblasts, EMT, hepatic stellate cells – HSCs), leads to excessive accumulation of extracellular matrix (ECM) and fibrosis progression. TGF-β1 and the Wnt/β-catenin pathway play essential roles in regulating these processes, and Galectin-3 emerges as both a biomarker and a potential therapeutic tar-get. In the kidney, TGF-β1 activates Smad2/3, promoting the expression of collagen and α-SMA. In the liver, activation of HSCs and their transformation into myofibroblasts are tightly regulated by TGF-β and interaction with the Wnt pathway. Functional and molecular connections between the liver and kidneys suggest that fibrosis in one organ may influence the progression of fibrosis in the other. Understanding the com-mon and interconnected mechanisms of hepatic and renal fibrogenesis opens the door to innovative therapeutic strategies. Inhibition of the TGF-β/SMAD, Wnt/β-catenin pathways, and Galectin-3 may provide a dual antifibrotic effect, potentially limiting fibrosis progression in both the liver and the kidneys.

Фиброз представляет собой конечную стадию хронического воспалительного процесса, способствуя развитию тяжёлых органных дисфункций. Миофибробласты, активируемые посредством сложных механизмов (TGF-β/SMAD, Wnt/β-катенин), играют ключевую роль в прогрессировании фиброза как в почках, так и в печени. Кроме того, эпителиально-мезенхимальный переход (EMT) и повышенная экспрессия галектина-3 усиливают профибротические реакции. Был проведён библиографический обзор статей, индексированных в базе данных PubMed, касающихся почечного и печёночного фиброза. Активация миофибробластов, происходящих из различных источников (резидентные фибробласты, EMT, звездчатые клетки печени - HSC), приводит к чрезмерному накоплению внеклеточного матрикса (ECM) и прогрессированию фиброза. TGF-β1 и путь Wnt/β-катенин играют важную роль в регуляции этих процессов, а галектин-3 выделяется как биомаркер и потенциальная терапевтическая мишень. В почках TGF-β1 активирует Smad2/3, способствуя экспрессии коллагена и α-SMA. В печени активация HSC и их превращение в миофибробласты строго контролируются TGF-β и взаимодействием с путём Wnt. Функциональные и молекулярные связи между печенью и почками позволяют предположить, что фиброз одного органа может способствовать прогрессии фиброза в другом. Понимание общих и взаимосвязанных механизмов фибро-генеза печени и почек открывает перспективы для разработки инновационных терапевтических стратегий. Ингибирование путей TGF-β/SMAD, Wnt/β-катенин и галектина-3 может обеспечить двойной антифибротический эффект, потенциально ограничивая прогрессию фиброза в обоих органах.
