Abstract:
The activity of sympathoadrenergic system was studied in 59 patients with mitral valvular disease (middle age 57,8±1,4 years). The patients were divided into two groups: the first group included 24 patients with middle age of 47,8±2,5 years and the second one included 35 patients from 60 to 75 years (mean age 66,2±1,1 years). The catecholamines were determined in 24 hour urine, according to patients' age, stage of heart failure, type of mitral valvular disease (the prevalence of regurgitation or stenosis) and life expectation after modern treatment. No association between the age and the level of catecholamines was revealed. The decrease of the reserve potentialities of sympathoadrenergic system was marked in severe stages of heart failure. L-DOPA test can estimate the status of sympathoadrenergic system and can be used in the prognosis of the evolution of mitral heart disease.
A fost explorat sistemul simpatoadrenergic (DA, NE, E şi AHV) într-o porţie de urină nictemerală la 59 de bolnavi cu valvulopatii mitrale asociate postreumatismale (vârsta medie 57,8±1,4 ani). Toţi pacienţii au fost repartizaţi în 2 loturi: până la 60 de ani erau 24 bolnavi cu vârsta medie de 47,8±2,5 ani, iar în lotul doi au fost 35 de pacienţi de la 60 până la 75 ani cu vârsta medie de 66,2±1,1 ani. Toate catecolaminele s-au studiat în funcţie de vârsta pacienţilor, stadiul de insuficienţă cardiacă cronică, tipul valvulopatiei mitrale asociate şi pronosticul vieţii bolnavilor după tratamentul modern.
S-a constatat că diferenţele în funcţie de vârstă nu sunt esenţiale, cu excepţia creşterii nesemnificative a dopaminei şi a norepinefrinei la vârstnici, epuizarea sistemului simpatoadrenergic este proporţională cu creşterea stadiului de ICC şi veridic mai pronunţată la bolnavii cu predominarea insuficienţei mitrale. Testarea specială cu 500 mg L-DOPA la debutul şi în finalul tratamentului complex dezvăluie suplementar criteriile clinico-biochimice prin epuizarea sau creşterea dopaminei şi a norepinefrinei din stadiile de insuficienţă cardiacă cronică, iar creşterea dopaminei numai de 10-12 ori după tratamentul modern poate constitui un pronostic nefavorabil pentru viaţa acestor bolnavi. Folosirea acestui test ne oferă noi posibilităţi de explorare a sistemului simpatoadrenergic în clinica valvulopatiilor cardiace.
