Importanţa factorului genetic în hipertensiunea arterială esenţială a copilului

Abstract

Essential arterial hypertension is a multifactorial disease that is the result of the interaction of genetic and environmental factor. Studying of molecular-genetic markers is advisable in childhood, because with age the values of the arterial tension are influenced by the environmental factors. But at the same time determination of the molecular-genetic markers can help us detect the children with high risks of arterial hypertension development and as a consequence possible complications. Effective prophylaxis on the early stage of the disease in comparison with the treatment of the formed pathology will have higher medical and social-economic effect. This article emphasis the role of the genetic factor in initial arterial hypertension at children basing on the accessible sources, focusing on the hereditary anamnesis and genetic polymorphism f the renin angiotensin aldosterone system and NOS endothelial synthase. In spite of the fact that in therapeutic literature dedicated to adults studying special attention is paid to genetic polymorphism of Initial arterial hypertension, in paediatrics there is a relatively small amount of investigations (more over they include a limited number of children) which examined the contribution of the genes to the phenotype of the arterial hypertension at children. They are often contradicting.

Первичная артериальная гипертензия является многофакторным заболеванием, результатом взаимодействия генетических факторов и факторов внешней среды. Исследование генетического фактора в детском возрасте обосновано, потому как с возрастом на уровень артериального давления все чаще влияют факторы окружающей среды. В то же время, определение молекулярно-генетических маркеров позволит нам выделить детей с высоким риском развития артериальной гипертензии и, как следствие, возможные осложнения. Эффективная профилактика на ранних стадиях болезни в сравнении с лечением уже сформированной патологии обеспечит положительный медицинский и социально-экономический эффект. Эта статья посвящена роли генетического фактора в развитии артериальной гипертензии у детей, основана на имеющейся литературе, с акцентом на наследственном анамнезе, генетическом полиморфизме ренина-ангиотензин-альдостероновой системы и эндотелиальной NOS синтазы. В сравнении с терапевтической литературой, где уделяется особое внимание генетическому полиморфизму ренина-ангиотензин-альдостероновой системы взрослого, а также системе эндотелиальной NOS синтазы, в педиатрии существует относительно небольшое количество исследований, часто с противоречивыми заключениями, которые рассмотрели вклад этих генов в фенотипическом выражении артериальной гипертензии в детской популяции.
