Modern approaches in the treatment of hand and finger osteoarthrosis

Abstract

Different methods of treatment of hand and finger osteoarthrosis, based on the modern literature, corresponding guidelines (ACR – American College of Rheumatology, OARSI – Osteoarthritis Research Society International and EULAR – the European League Against Rheumatism) and existing clinical standards are presented. PubMed database has been consulted and the review of 47 articles relevant to the studied theme has been made. Respectively, the modern management of hand and finger osteoarthrosis includes non-medicamentous, medical and surgical treatment. Analgetics administration, separate injections of intraarticular corticosteroids and regular ones of chondroprotective remedies as well as physiotherapy remain the cornerstone of the hand and finger conservative therapy of osteoarthritis. Also, there are a number of “alternative” therapeutic options, which can be effective for some patients, especially with secondary osteoarthrosis. Surgery is generally applicable in advanced cases associated with massive destruction of the joints surfaces, which are beyond the reach of conservative treatment. Although there are a lot of proposed methods of treatment, at the moment there is no “golden standard” treatment for such patients, and real life therapy should be based on the individual approach. In conclusion the author points out, that the treatment of hand and finger osteoarthritis is a quickly progressing research area, that has already presented several therapies, which can provide the relief of the disease manifestations and even slow down the disease development in some situations.

Представлены различные методы лечения остеоартроза руки и пальцев на базе информации, представленной в современной литературе, соответствующих гидах (ACR – Американский Колледж Ревматологов, OARSI – Международное Общество Исследования Остеоартрита и EULAR – Европейская Лига Против Ревматизма) и существующих клинических стандартах. Изучена медицинская база данных PubMed, на основе которой сделан обзор 47 статей по теме данной научной работы. Соответственно, современное ведение пациентов с оcтеоратрозом руки и пальцев включает в себя немедикаментозные, лекарственные и хирургические методы лечения. Назначение противоболевых препаратов, нерегулярные внутрисуставные инъекции кортикостероидов и регулярные – хондропротекторов вместе с физиотерапией представляют собой основу консервативного лечения остеоартроза. Также существуют «альтернативные» методы лечения, которые могут быть эффективны для некоторых групп пациентов, особенно с вторичным остеоартрозом. Хирургические вмешательства обычно показаны в запущенных случаях при значительном разрушении суставных поверхностей, которые не могут быть вылечены с помощью консервативного лечения. Несмотря на наличие большого количества лечебных методик, на данный момент не существует «золотого стандарта» лечения для данной группы пациентов и их практическое ведение требует индивидуального подхода. Лечение остеоартроза руки и пальцев является быстроразвивающейся областью науки, которая уже обеспечила появление нескольких лечебных методов, способных облегчить клинические проявления этого заболевания и иногда даже замедлить его развитие.
