Problemele actuale ale serviciului de expertiză medicală a vitalităţii din Republica Moldova şi strategia de dezvoltare în perspectivă

Abstract

Moldova’s ratification of UN Convention on the Rights of Persons with Disabilities by Law 166 of 09.07.2010 and adoption of the “Strategy of Inclusion of Persons with Disabilities (2010-2013)” by Law no. 169 of 09.07.2010, impose a series of measures the state has undertaken to respect and guarantee the rights of persons with disabilities. To facilitate the adjustments to the European and International standards, we contributed to an elaboration of strategy to create in Moldova a new vision of disability consistent with the international medical and social model. The model ensures the rights of persons with disabilities to medical, social and professional rehabilitation and inclusion. Several models were studied for assessing disability, such as: percentage determination of the degree of reduction and limitation, definition of special care needs according to the degree of complexity in performing daily activities, determination of the preserved working capacity of each person, etc. The procedure for disability determination is the first step, which brings people with disabilities into the social protection system and ensures a comprehensive assessment in order to direct the person to other services designed to promote inclusion. Emphasis is placed on the liaison between the primary care institution that sends the patient to medico-social assessment, the institution that determines the disability (RCMEV with its territorial subdivisions) and governmental institutions that provide the necessary support for medical and professional rehabilitation, for education and employment, and for accessing the necessary social services. Taking into consideration the deficiencies of the current system, the elaboration of the national model based on the medical and social expertise is in an active development phase and relies on the following prerequisites: 1. Strengthening the technical and material base of the institution and organizational review; 2. Creating a single information system for persons with disabilities; 3. Developing the criteria of disability determination for adults and children; 4. Training the staff involved in the reform implementation; 5. Ратификация Конвенции ООН о правах людей с ограниченными возможностями Республикой Молдова Законом № 166 от 09.07.10 и утверждение Стратегии об интеграции людей с недугами (2010-2013) Законом № 169 от 09.07.10 обязывает государство выполнить ряд мероприятий взятых на себя для соблюдения и гарантирования прав людей с ограниченными возможностями. Основываясь на этом, с целью приближения службы в соответствии с международными и европейскими стандартами был создан План действий (стратегия) по реформированию Службы врачебно-трудовой экспертизы жизнеспособности выбирая био-психо- социальную модель, основанную на Международной Классификации Функциональности (МКФ). Было изучено множество моделей определения недуга (инвалидности), из которых: определение процента и степени ограничения, определение специальных нужд исходя из степени ограничения возможностей, определение процента сохранения трудоспособности каждого индивидуума и т.д. Процедура определения недуга и трудоспособности – это первоначальный этап, который гарантирует ряд социальных мер защиты для каждого отдельного человека. Поэтому эта процедура требует комплексной оценки ввиду дальнейшего направления в соответствующие службы, содействующие социальной интеграции людей с недугами. Необходимо отметить, важность взаимосвязи между направившим учреждением (первичная медицинская сеть), учреждением определяющим недуг и трудоспособность (РКВЭЖ) и те государственные учреждения обеспечивающие социальную защиту и проведение реабилитационных мероприятий, внедрение в систему воспитания и трудовую деятельность, а также в сферу социальных услуг согласно социальным нуждам. Исходя из недостатков существующей системы, важно чтобы будущая национальная модель, для ее успешного внедрения на первоначальных этапах, включала в себя: 1. Улучшение материально- технической базы службы. 2. Создание информационной базы данных людей с недугами. 3. Разработка и составление медицинских и социальных критериев определения степени недуга у детей и трудоспособности у взрослых. 4. Обучение и профессиональная подготовка персонала для предстоящих реформ.
