Mecanismele etiopatogenetice implicate în dezvoltarea tiroditei autoimune: sinteză narativa de literatură

Abstract

Introducere. Tiroidită autoimună are o prevalenţă înaltă. Manifestările clinice ale hipotiroidiei de origine autoimună nu întotdeauna sunt ameliorate de tratamentul cu levotiroxină, lăsând, astfel, loc de dezbateri în ceea ce priveşte abordarea clinică a acestor pacienţi. în ultimul timp, se vorbeşte tot mai mult despre influenţa receptorilor TLR, a microbiotei asupra mecanismelor etiopatogenetice din tiroidită autoimună. Astfel, datele recente ar putea, în viitorul apropiat, schimba modul de abordare a pacientului cu tiroidită autoimună - de la testele diagnostice până la principiile de tratament. în acestă lucrare, ne-am propus sa facem review-ul datelor din literatură referitoare la mecanismele de dezvoltare ale tiroiditei au- toimune. Material şi metode. Pentru a selecta datele din literatură, am folosit baza de date PubMed, utilizând cuvintele cheie „autoimmune thyroiditis",„TLR",„microbiota". Astfel, a fost sintetizată informaţia despre etiopatogenia tiroiditei autoimune. Rezultate. Mecanismele patogenetice contemporane din tiroidită autoimună sunt mimicria moleculară, activarea martorului, apoptoza celulelor tiroidiene. Studiile recente evidenţiază rolul microbiotei şi al activării aberante a sistemului imun înnăscut în patogenia tiroiditei autoimune. Concluzii. Cunoaşterea noilor mecanisme etiopatogeneti- ce în tiroidită autoimună va oferi, în viitor, posibilitatea unei noi abordări diagnostice şi terapeutice a pacientului cu hipo-tiroidie.

Introduction. Autoimmune thyroiditis has a high preva- lence. Clinical manifestations of hypothyroidism due to Hashimoto thyroiditis are not always improved by levothyroxine treatment, thus causing debate on the clinical approach of these patients. Lately, researches in the field suggest Toll- like receptors, the microbiota influence the etiopathogenetic mechanisms of autoimmune thyroiditis. Thus, recent data autoimmune thyroiditis. In this paper we intend to review the literature in terms of mechanisms of development of autoimmune thyroiditis. Material and methods. The PubMed database was used in order to select the data from the literature, using the key- words “autoimmune thyroiditis", “TLR", “microbiota". Thus, the information containing the data on the etiopathogenesis of au- toimmune thyroiditis was retained. Results. Contemporary pathogenetic mechanisms of auto- immune thyroiditis are molecular mimicry, bystander activa- tion, and thyroid cell apoptosis. Recent studies highlight the role of microbiota and aberrant activation of the innate im- mune system in the pathogenesis of autoimmune thyroiditis. Conclusions. Knowing the new etiopathogenetic mechanisms in autoimmune thyroiditis will in the future provide the possibility of a new diagnostic and therapeutic approach of the patient with hypothyroidism.
