AbstractIntroducere: Infecţia plaselor sintetice în chirurgia herniilor abdominale poate necesita inlaturarea plasei şi este asociată cu un risc înalt de recurenţă. Material si metode: Pe parcursul a doi ani în Clinica Chirurgie Generală au fost trataţi 574 pacienti cu hernii abdominale. Bărbaţi – 367 (63,9%), femei – 207 (36,1%). Indexul masei corporale (IMC) mai mare de 30 s-a determinat la 112 (19,5%). Pentru întărirea defectelor peretelui abdominal la 559 (97,4%) pacienţii s-a utilizat plasa de polipropilenă. În mod urgent au fost operaţi 32 (5,7%) pacienţi, programat – 527 (94,3%). Patologie concomitentă au avut 307 (54,9%) pacienţi. În 136 (24,3%) cazuri plasa a fost inserata underlay, în 406 (72,6%) – inlay, iar în 17 (3,0%) – onlay. Rezultate: Infecţia plăgii chirurgicale (IPC) a fost înregistrată în 38 (6,8%) cazuri, clasificată ca: IPC superficială (22 – 3,9%) şi IPC profundă (16 – 2,9%), inclusiv cu infectarea plasei în 10 (1,9%) cazuri. IPC s-a stabilit la 11 (34,4%) pacienţi operaţi în mod urgent şi la 21 (4,0%) – în mod programat. La bonavii cu IPC s-a constatat prevalenta IMC peste 30 (OR-1.1194, 95% CI: 0,5530 to 2,2657, Z 0,314, P=0,7539) şi a comorbidităţilor (OR-1,1373, 95% CI:0,6370 to 2,0307, Z 0,435, P=0,6634). Inserarea plasei underlay, inlay sau onlay nu a influiențat statistic semnificativ apariţia IPC. Debridarea chirurgicală a plăgii s-a efectuat la 22 (78,6%) pacienti din cei cu IPC. Înlaturarea partială sau completă a plasei a fost necesara la 9 bolnavi. Concluzii: Rata de infectare a plaselor sintetice este mai înaltă în cazurile de intervenţii chirurgicale de urgenţă și la pacienții cu IMC elevat. Infecţia plăgii chirurgicale se asociază cu un risc major de infectare a plaselor.
Introduction: Infection of synthetic mesh in abdominal hernia surgery could require mesh removing, which is associated with a high risk of recurrence. Material and methods: During two years’ period, 574 patients with abdominal hernias were treated in the Department of General Surgery. Men – 367 (63,9%), women – 207 (36,1%). In 112 (19,5%) patients the body mass index (BMI) were higher than 30. For enforcement of abdominal wall defects in 559 (97,4%) patients used polypropylene mesh with macrospore over 10 mcm. In emergency setting were operated 32 (5,7%) patients and in elective mode – 527 (94,3%). Comorbidities had 307 (54,9%) patients. In 136 (24,3%) cases the mesh was placed underlay, in 406 (72,6%) – inlay, and in 17 (3,0%) – onlay. Results: Surgical site infection (SSI) was registered in 38 (6,8%) cases, classified as: superficial SSI (22 – 3,9%) and deep SSI (16 – 2,9%), including mesh infection in 10 (1,9%) cases. SSI were determined in 11(34,4%) patients, who undergone emergent surgery, and in 21(4,0%) – operated in elective mod. In patients with SSI the prevalence of body mas index over 30 (OR-1.1194, 95% CI: 0,5530 to 2,2657, Z 0,314, P=0,7539), and comorbidities (OR-1,1373, 95%CI:0,6370 to 2,0307, Z 0,435, P=0,6634) were noted. The placement of mesh underlay, inlay or onlay does not influence statistical significant the occurrence of SSI. Surgical debridement of wounds performed in 22 (78,6%) from cases with SSI. Partial or complete removal of the mesh was necessary in 9 patients. Conclusions: The incidence of synthetic mesh infection is higher in emergency surgery setting, as well as in patients with increased BMI. The surgical site infection is associated with a major risk of prosthetic mesh contamination.
