Procesul displastic se caracterizează prin apariţia modificărilor degenerativ-distrofice în elementele SVM, care îi diminuează rezistenţa.
Aceasta din urmă conduce la instabilitate realizată preponderent sub formă de deviaţii patologice ale vertebrelor, deformaţii excesive
ale inelului fibros şi ale corpilor vertebrali. La oricare din aceste stadii de evoluţie a procesului displastic, prin ameliorarea capacităţilor
portante ale coloanei, se poate jugula instabilitatea acesteia. Aspectele de principiu ale tacticii curative în raport cu maladiile displastice
ale coloanei se pot elucida prin analiza a trei poziţii de suport: caracterul modificărilor patologice; prezenţa şi expresia tulburărilor neurologice; statutul clinico-radiologic şi biomecanic al coloanei vertebrale. Concepţiile teoretice elaborate oferă posibilitatea de a formula unele
criterii de aspect tactic şi strategic în abordarea curativă a instabilităţii vertebrale displastice: pentru instabilitatea complicată primul gest
curativ este cel ce tinde spre anularea sau prevenirea aprofundării complicaţiilor produse deja; pentru lichidarea instabilităţii se impune
majorarea capacităţilor portante ale segmentului vertebral motor (a corpilor şi discurilor intervertebrale). Soluţiile tactice de această
orientare sunt destul de variate şi includ un întreg arsenal de procedee conservative şi operative de tratament. Dintre cele chirurgicale
cea mai argumentată este spondilolisteza posterioară. De aceea, este mai bine ca bolnavii să fie operaţi la vârste tinere şi să se realizeze
operaţii stabilizatoare, fără a interveni pe canalul spinal, prevenind stenozarea acestuia, decât să se opereze pacienţi vârstnici cu variante
de proces displastic stenozant.

instability development which is realized mainly in the form of pathological vertebras moving, superfluous deformations of a fibrous ring
and backbone deformation. On any of development stages of displastical process, increase of the broken bearing ability of a backbone can
stop its instability. Principle questions of medical tactics at displastical vertebral diseases are based on the three main positions analysis:
character of pathological changes; presence and expressiveness of neurological infringements; clinic-radiological and biomechanical
vertebral conditions. The resulted theoretical constructions give the chance, to formulate a number of positions in aspect of strategy
and treatment tactics at displastical vertebral instability. At the complicated instability on the foreground of medical actions necessity
of liquidation or the prevention of aggravation of complications enters, for instability liquidation increase of bearing ability vertebral
– impellent segment (bodies of vertebras and intervertebral disks) is necessary. This direction tactical decisions are various and include
an arsenal conservative, and operative techniques of treatment. From surgical methods it is most proved posterior spine fusion. From this
view point, it is better to operate patients at young age and to apply stabilizing operation without intervention on vertebral channel and
to warn, it stricture formation, than to carry out decompression - stabilizing operation at adult patients with stricture formation variant
displastical process.
