Introducere. Rezultatele tratamentului formelor distructive ale erizipelului rămân nesatisfăcătoare, ceea ce ne
impun să realizăm noi metode în tratament.
Scopul. Ameliorarea rezultatelor tratamentului formelor distructive ale erizipelului.
Material şi metode. Studiul cuprinde 52 de pacienţi: 35-cu complicaţii locale minore şi 17-cu complicaţii locale
majore pe o perioadă de 10 ani (2006-2015). B-15, F-37, vârsta medie 58,7 ani. Tratamentul chirurgical al
formelor distructive minore s-a limitat la deschiderea bulelor pustulizate şi abceselor mici, excizia necrozelor
superficiale limitate ale pielii şi tratamentul lor cu betadină,unguente hidrosolubile; Toţi cei 17 pacienţi cu
forme distructive majore au fost supuşi debridării chirurgicale agresive: 9 din ei mai departe au primit tratament
tradiţional, iar 8 VAC aspiraţie. Defectele pielii au fost închise cu autodermă. La 6 pacienţi cu plăgi circulare
pentru protejarea lambourilor s-a instalat aparatul Ilizarov. Eficacitatea tratamentului a fost apreciată după
gradul de reducere a edemului şi numărul de zile pat.
Rezultate. Utilizarea VAC aspiraţiei în tratamentul formelor distructive majore ale erizipelului a permis
micşorarea termenilor de pregătire a plăgii pentru închiderea ei cu autodermă de la 44,31 ± 2,46 până la 22,57
± 3,17 zile ( P < 0,001, t = 5,42 ), iar durata tratamentului s-a redus de la 53,44 ± 4,74 până la 30,0 ± 3,43 zile (
P < 0,01, t = 4,0).
Concluzii. Debridarea chirurgicală agresivă ,VAC aspiraţia reduce semnificativ durata tratamentului formelor
distructive majore ale erizipelului.
Introduction: The treatment results of destructive forms of erysipelas (DFE) remain unsatisfactory, which us
impose to realize the new treatment methods.
Aim: To improve results of treatment DFE.
Materials and Methods: The study includes 52 patients: 35 with minor local complications and 17 with major
local complications in a period of 10 years (2006-2015). Men 15, female 37 , medium age 58,7 years. The surgical
treatment of destructive minor forms was limited on opening of pustule and minor abscesses, the excision of superficial limited necroses of teguments and use of betadine and hydrosoluble unguents. All the patients with
major destructive forms were treated with surgical aggressive debridation:9 had a traditional treatment, and
8 VAC aspiration. The defects of tegument were closed with autoderma. In 6 cases with circular wounds was
installed the Ilisarov apparatus ,for protection of skin flaps. The efficacy of treatment was appreciated after the
degree reduced of edema and number of days in the hospital.
Results: The use of VAC in treatment of major DFE permit to reduce the terms from prepare wound closing
with autoderma from 44,31 ± 2,46 till 22,57 ± 3 ,17 days ( P < 0,001, t = 5,42 ) and the duration of treatment was
reduced from 53,44 ± 4,74 to 30,0 ± 3,43 days ( P < 0,01, t = 4,0)
Conclusions: The aggressive surgical debridement, VAC aspiration significant reduced the duration of treatment
of major destructive forms of erysipelas.
