Introducere: Pacienții cu traumatism asociat toracoabdominal care sunt hemodinamic instabili sunt o provocare majoră,
frecvent se impune necesitatea soluției chirurgicale: laparotomia și/sau toracotomia. În acest scenariu orice ezitare se
penalizează cu mortalitate, astfel consecutivitatea intervențiilor chirurgicale poate fi vitală.
Scopul: Argumentarea tacticii chirurgicale la pacienții cu traumatism toracoabdominal și hemodinamică instabilă.Material și metode: S-a realizat un studiu retrospectiv la baza Institutului de Medicină Urgentă (Chișinău) care a inclus
traumatizați cu plăgi toracoabdominale și leziuni de diafragm fără implicarea cordului, hemodinamic instabili. Au fost identi ficate
35 fișe de observație timp de 12 ani.
Rezultate: Din cei 35 pacienți unul a decedat la scurt timp de la adresare fără a se reuși intervenția chirurgicală. În rest, 34
traumatizați au suportat laparotomie, iar 8 dintre aceștia – și toracotomie. Consecutivitatea intervențiilor a fost, în primul rând:
laparotomia – 5, toracotomia – 3. Durata medie a perioadei preoperatorii intraspitalicești – 29,6±2,8 minute. Au fost constatate:
leziuni de organe cavitare și/sau parenchimatoase – 16, organe parenchimatoase – 17, izolat diafragma – 2 pacienți.
Hemoragie >1 litru au avut 26 (74,3%) pacienți. Sursa majoră de hemoragie localizată intratoracic a fost la 4 traumatizați, în rest
fiind intraabdominală. Mortalitatea generală – 8 (22,8%).
Concluzii: Sursa majoră de hemoragie la această categorie de traumatizați, mai frecvent, are localizare abdominală;
majoritatea covârșitoare având leziuni intraabdominale majore. La pacienții cu traumatism toracoabdominal hemodinamic
instabili de primă intenție este aplicarea drenului toracic și în lipsa exteriorizării unei hemoragii majore se va interveni prin
laparotomie exploratorie.
Introduction: Patients with penetrating thoracoabdominal trauma and hemodynamic instability are a major challenge for trauma
teams, usually require surgical approach and impose dilemma between laparotomy or/and thoracotomy. When the time is very
tight, consecutiveness of operations can be critical for patient survival.
The aim of study: Argumentation of surgical management of penetrating thoracoabdominal trauma with hemodynamic
instability.
Material and methods: Eligibility criteria were: penetrating thoracoabdominal trauma, hemodynamic instability, trauma of
diaphragm but without trauma of heart. A total of 35 patients presented to Chisinau Emergency Hospital during a 12-years
period were retrospectively evaluated. Either laparotomy, thoracotomy or both of them were performed. Outcomes of the
management were analyzed.
Results: Among 35 patients: 1 died before any surgical procedure, but 34 underwent laparotomy and 8 of them – thoracotomy.
Consecutiveness of surgical procedures was, firstly: laparotomy – 5, thoracotomy – 3. The average preoperative time –
29.6±2.8 minutes. They had injury of: hollow viscera – 16, parenchymal – 17, only diaphragm – 2. A total of 26 (74.3%) patients
had bleeding >1 L. The main source of bleeding was localized in thorax just in 4 patients. The overall mortality was 8 (22.8%)
cases.
Conclusions: The main cause of bleeding is usually within abdomen, majority of patients having severe intraabdominal injuries.
Therefore, in trauma units where immediate thoracotomies are not part of the daily routine, for patients with penetrating
thoracoabdominal trauma and hemodynamic instability first line procedure is tube thoracostomy and if there are no signs of
major bleeding – perform laparotomy.
