Abstract:
Background. The 58-year-old patient had movement disorders such as dysmetria, ataxia, dysarthria and
tremor. Objective of the study. Identifying the pathology that lead to the appearance of clinical
manifestations with genesis and location difficult to establish. It is necessary to make a differential
diagnosis to establish the exact cause of the disabilities in the given patient. Material and Methods. To
establish the final diagnosis are used the patient's history, laboratory and imaging investigations over 5
years, but also dynamic monitoring of the patient. Results. FXN gene test and the serum cooper dosage
exclude Friedrich Ataxia and Wilson disease. The most probable diagnostics are Multisystem
atrophy(MSA) and Spinocerebellar ataxia (SCA). But if we look at the tremor we can notice that it is
present both at the rest and during moving. This finding led us to assume that the lession is present both
cerebellum and basal ganglia that is clinically relevant for Multisystem atrophy. Also the pacient shows
signs of disautonomia manifested by decreased blood pressure in orthostatic position that represent one
of the MSA criteria. Conclusion. The most likely diagnosis is Multisystem atrophy but monitoring in
dynamics both clinicaly and paraclinicaly is very important in the establismhment of the definitive
diagnosis.
Introducere. Pacientă de 58 de ani, tulburări de mișcare precum: dismetrie, ataxie, dizartrie și
tremor. Scopul lucrării. Elucidarea patologiei care a dus la apariția a numeroase manifestări clinice cu
geneză și localizare dificil de stabilit. Este necesar de a efectua un diagnostic diferențial pentru stabilirea
cauzei exacte a tulburărilor la pacienta dată. Material și Metode. Pentru stabilirea diagnosticului final
sunt folosite istoricul pacientei, investigațiile de laborator si imagistice pe parcursul a 5 ani, dar si
monitorizarea în dinamică a pacientei. Rezultate. Investigațile la gena FXN, dar și dozarea cuprului
seric exclude Ataxia Friedrich și Boala Wilson. Un diagnostic mai exact ar fi atrofia multisistemică
(MSA) sau atrofia spinocerebelară (SCA). Dacă analizăm tremorul, însă observăm că este un tremor atât
de mișcare cât și de repaus. Aceasta indică o leziune atât la nivel cerebral, cât și la nivelul ganglionilor
bazali, un criteriu în favoarea MSA. De asemenea, pacienta prezintă semne de disautonomie manifestată
prin scăderea tensiunii arteriale în poziție ortostatică, care poate fi considerat, de asemenea, un criteriu
în favoarea MSA. Concluzii. Diagnosticul cel mai probabil, este atrofie multisistemică (MSA), dar
evaluarea în dinamică atât clinic, cât și imagistic este esențială în stabilirea diagnosticului definitiv.
