Particularităţile organizatorice ale serviciului de asistenţă medicală primară în municipiul Chişinău

Abstract

The data regarding existing structure of the primary health care system in Chisinau municipality are presented in this publication comparison with the regions of the Republic of Moldova and in the reorganization under the implementation of mandatory health care insurances. A meta-analysis has been carried out of existing legal documents establishing organizational and activity principles of public medical sanitary institutions within primary health care system. Organizational particularities of the PHC (Primary Health Care) service in Chisinau Municipality, as well as the impact of implementation on the mandatory health care insurances have been underlined in 3 focus groups with participation of 63 specialists in family medicine and I and II degree managers. The list of impediments has been developed. The lack of comprehensive assessment criteria of the relevant sector activity within the local health system and quantitative estimation of the quality implies a high degree of subjectivity in estimations, inducing uncertainties in decisions related to improvement measures and error correction.

В настоящей статье представлены данные о действующей структуре первичной медицинской службы в муниципии Кишинэу в сравнение с таковой и районах Республики Молдова и её реорганизация в условиях внедрения обязательного медицинского страхования. Был проведен мета-анализ законодательной базырегламентирующей принципыорганизации и деятельности учреждений первичной медицинской службы. В работе 3 фокус-групп, с участием 63 семейных врачей и менеджеров I и II уровня, были выявлены организационные особенности первичной медицинской службы в муниципии Кишинэу, а также влияние внедрения обязательного страхования в медицине. Был выявлен список факторов, которые затрудняют их деятельность. Отсутствие критериев для комплексной оценки деятельности первичной медицинской службы и для количественной оценки качества оказанных услуг, способствует повышению доли субьективизма проводимых экспертных оценок, снижая эффективность принятых мер по оптимизации деятельности и исправлению ошибок.
