Respectarea standardelor de supravegherea medicală în condiţii de ambulatoriu la copiii decedaţi de pneumonia în Moldova

Abstract

Background. Reducing infant and child mortality is a priority for Moldova. Significant reserves of its solutions lie in improving the effectiveness of primary health care services. Aim. Retrospective assessment of compliance with “Standards of medical supervision of the children of 0-18 years” in the children who died from pneumonia. Materials and Methods: In a retrospective cohort study was carried out confidentiality audit of 44 death cases of children from 29 days to 5 years of pneumonia. The findings were compared with the results of the study “Monitoring the implementation... ... Standards of supervision of children 0-18 years on an outpatient basis” (2014) conducted in the Republic of Moldova with the participation of 500 children. Statistical analysis was performed using the program EpiInfo 7.1.5.2. Results: The level with compliance of the standard of preventive medical supervision (home visiting, regular visits to the family doctor, the mandatory consultation of specialists, and laboratory tests) in children with fatal pneumonia was statistically lower than in the general pediatric population (p <0.05). Conclusions: Children who die from pneumonia were under inadequate medical supervision. Strict compliance with the applicable “Standards observation of children 0-18 years on an outpatient basis” will contribute to a reduction of infant and child mortality.

Введение. Снижение детской и младенческой смертности является приоритетной проблемой для Молдовы. Значительные резервы её решения кроются в повышении эффективности работы первичного звена медицинской службы. Цель. Ретроспективная оценка соблюдения «Стандартов наблюдения детей 0-18 лет в амбулаторных условиях» у детей, умерших от пневмонии. Материалы и методы: В рамках ретроспективного когортного исследования был проведен конфиденциальный аудит 44-х случаев смерти детей в возрасте от 29 дней до 5 лет от пневмонии. Полученные данные были сопоставлены с результатами исследования «Мониторинг процесса внедрения… …Стандартов наблюдения детей 0-18 лет в амбулаторных условиях» (2014), проведенного в РМ с участием 500 детей. Статистический анализ проводился с использованием программы EpiInfo 7.1.5.2. Результаты: уровень соблюдения стандартов профилактического медицинского наблюдения (визиты медперсонала на дом, регулярное посещение семейного врача, обязательные консультации специалистов и лабораторные исследования) у детей с фатальным исходом пневмонии был статистически ниже, чем в общей педиатрической популяции (р<0,05). Выводы: дети, умершие от пневмонии, находились под недостаточным медицинским наблюдением. Строгое соблюдение действующих «Стандартов наблюдения детей 0-18 лет в амбулаторных условиях» будет способствовать снижению младенческой и детской смертности.
