Background. Postoperative dysphagia (PD) following a laparoscopic fundoplication (FL) is a common
and self-limiting phenomenon in the first postoperative weeks. However, 8-35% of patients suffer from
persistent postoperative dysphagia (PPD) lasting more than 12 weeks. Objective of the study. To assess
the therapeutic options in patients with DPP after FL, operated for gastroesophageal reflux disease
(GERD) and hiatal hernia (HH). Material and Methods. The studied group included 160 patients with
GERD, operated in the period 2012-2019 in the Semiotics of General Surgery Clinic no.3. Women were
99 patients (61.8%), men - 61 patients (38.2%). The age of the patients ranged from 19 to 76 years (mean
52 years). Most patients underwent laparoscopic Nissen-Rossetti fundoplication. Results. 70 (46%)
patients presented with PD, which resolved without specific treatment within up to 4 weeks. In 2 (1.3%)
patients, PPD was noticed, lasting more than 3 months, which required postoperative curative
interventions. 1 case was resolved by repeated endoscopic balloon dilation, performed on the 8th and
11th postoperative day, with a positive effect at 3 years of postoperative follow-up. In another case, due
to the failure of endoscopic treatment methods, the DPP indicated reoperation by open approach 3
months after the primary LF - Nissen redone on a 54Fr probe. At 7 years of postoperative follow-up,
both the control of GERD symptoms and the lack of dysphagia were noticed. Conclusion. PD is a
specific complication after FL, with a tendency to self-limitation within 4 weeks. However, in the case
of PPD - endoscopic treatment methods are used, and their failure may require a redo surgery.
Introducere. Disfagia postoperatorie (DP) în urma unei fundoplicații laparoscopice (FL) este un
fenomen comun și autolimitant în primele săptămâni postoperatorii. Totuși, 8-35% dintre pacienți suferă
de disfagie postoperatorie persistentă (DPP) cu o durată mai mare de 12 săptămâni. Scopul lucrării.
Aprecierea posibilităților curative la pacienții cu DPP după FL, operați pentru boala de reflux
gastroesofagian (BRGE) și hernie hiatală (HH). Material și Metode. Lotul studiat a inclus 160 de
pacienți cu BRGE, operați în perioada 2012-2019 în Clinica Chirurgie generală-semiologie nr.3.
Femeile au constituit 99 paciente (61,8%), bărbații – 61 pacienți (38,2%). Vârsta pacienților a variat
între 19 și 76 de ani (media 52 ani). Majoritatea pacienților au suportat fundoplicație Nissen-Rossetti
laparoscopică. Rezultate. 70 (46%) pacienți au prezentat DP, care s-a rezolvat fără tratament specific,
în decurs de până la 4 săptămâni. La 2 (1,3%) pacienți s-a remarcat DPP, cu durata de peste 3 luni, care
au necesitat intervenții curative postoperatorii. 1 caz a fost rezolvat prin dilatare endoscopică repetată
cu balon, efectuată la a 8-a și a 11-a zi postoperator, cu efect pozitiv la 3 ani de urmărire postoperatorie.
În alt caz, la eșecul metodelor endoscopice de tratament, DPP a impus reoperația prin abord deschis la
3 luni de la cea primară – refacerea Nissen pe un sondă de 54Fr. La 7 ani de urmărire postoperatorie sa remarcat atât controlul simptomelor BRGE, cât și lipsa disfagiei. Concluzii. DP reprezintă o
complicație specifică în urma FL, cu tendința spre autorezolvare în decurs de 4 săptămâni. Totuși, în
cazul DPP sunt necesare metode endoscopice de tratament, iar la eșecul acestora poate fi impusă operația
repetată.
