Posttraumatic pneumothorax

Abstract

Background: Pneumothorax is a medical-surgical emergency that consists in transforming the virtual pleural cavity into a real cavity, by accumulating air between the two pleural sheets. Objective of the study. Knowledge and application of first aid measures in post-traumatic pneumothorax in the prehospital and hospital stage. Material and Methods. To achieve the proposed goal, we analyzed the literature review between 2001-2014, using other bibliographic sources, including the Medical Scientific Library of USMF "Nicolae Testemitanu", data from the electronic libraries Medline, Medscape. Results.Intraoperative pneumothorax on the monoventilated lung (thoracic surgery) is a maximum emergency. Undiagnosed and undrained air in the pleura can lead to the development of hypertensive pneumothorax with faster exitus than in the case of ventilation of two lungs. The gesture of maximum urgency consists in creating a pleuro-pleural window (open pleura - pleura with hypertensive pneumothorax) through the bottom of the anterior cost-mediastinal sac, in front of the large vessels. The air is evacuated under pressure, after which the patient's respiratory and hemodynamic condition improves. Conclusion. Treatment: ensuring complete expansion of the lung + prevention of recurrences. Management is different depending on the trauma. In pneumothorax pleural drainage and oxygen therapy. In pyopneumothorax: pleural drainage + antibiotic therapy. In pneumomediastinum conservative attitude.

Introducere. Pneumotoraxul este o urgenţă medico-chirurgicală care constă în transformarea cavității pleurale virtuale într-o cavitate reală, prin acumulare de aer între cele două foițe pleurale. Scopul lucrării. Cunoaşterea şi aplicarea măsurilor de prim ajutor în pneumotorax posttraumatic, în etapa prespitalicească şi spitalicească. Material și Metode. Pentru realizarea scopului propus am efectuat analiza review-ului literaturii, între anii 2001-2014, utilizând și alte surse bibliografice, dintre care pe cele ale Bibliotecii Științifice Medicale ale USMF „Nicolae Testemiţanu”, date ale bibliotecilor electronice Medline, Medscape. Rezultate. Pneumotoraxul intraoperator apărut pe plămân monoventilat (chirurgia toracică) reprezintă o urgență maximă. Nediagnosticarea şi nedrenarea aerului din pleură poate duce la dezvoltarea unui pneumotorax hipertensiv, cu exitus mai rapid decât în cazul ventilaţiei a doi plamani. Gestul de urgență maximă constă în crearea unei ferestre pleuro-pleurale (pleura deschisa – pleura cu pneumotorax hipertensiv) prin fundul de sac costo-mediastinal anterior, prin faţa marilor vase. Aerul se evacueaza sub presiune, după care condiția respiratorie şi hemodinamica pacientului se ameliorează. Concluzii. Tratament: asigurarea expansiunii complete a plămânului + prevenirea recidivelor. Managementul este diferit, în funcţie de traumatism. În pneumotorax drenarea pleurală şi oxigenoterapie. În piopneumotorax: drenaj pleural + antibioticoterapia. În pneumomediastin atitudine conservatoare.
