Implanto-prosthetic treatment with early loading of dental implants

Abstract

Background. In recent years, implant dentistry has seen unparalleled technological advances, both surgical and prosthetic. Simultaneously with this technological revolution in the field of implant dentistry, the literature abounds more and more in longitudinal studies. Objective of the study. Study and analysis of the particularities of the implant-prosthetic treatment with early functional loading to reduce the loading terms of endosseous dental implants. Material and Methods. Lately, more and more followers of the method of early loading of dental implants with the restoration of the lost functions of edentulous patients are appearing among implantologists. The problems of dynamic processes occurring on the surface of dental implants and their importance in terms of biomechanics have been addressed by many scientists and it has been established that primary implant. Results. Tawse-Smith et al., Reported 71% survival rate in the group of early loading of dental implants. However, Payne et al. reported periotest values after 1 year in the early loading group were - 4.12 compared to -3.84 at the beginning of the first year; after 2 years these values were -4.52 -4.74 and, consequently, the values increase, which indicates a satisfactory stability, as well as a good osseointegration. Conclusion. In the practical implementation of early functional loading protocols, they led to the primary stability of the dental implant with their early loading after 3-4 weeks after implantation, which favored the faster restoration of the aesthetic and functional appearance.

Introducere În ultimii ani, stomatologia implantară a cunoscut progrese tehnologice de neegalat, atât chirurgicale, cât şi protetice. Concomitent cu această revoluţie tehnologică în domeniul stomatologiei implantare, literatura de specialitate abundă tot mai mult în studii longitudinale. Scopul lucrării. Studiul şi analiza particularităţilor tratamentului implanto-protetic, cu încărcare funcţională precoce, pentru micşorarea termenelor de încărcare a implantelor dentare endoosoase. Material și Metode. În ultimul timp, printre implantologi, apar tot mai mulți adepți ai metodei de încărcare precoce a implantelor dentare cu restabilirea funcțiilor pierdute ale pacienților edentați. De problemele proceselor dinamice, care se întâmplă la suprafața implantelor dentare și importanța lor din punct de vedere al biomecanicii, s-au ocupat mulți savanți, fiind stabiliţi că implanţi primari. Rezultate. Tawse-Smith et al., a raportat 71% rata de supraviețuire, la lotul de încărcare precoce a implantelor dentare. Cu toate acestea, Payne et al. a raportat valorile periotest, după 1 an în grupul cu încărcare precoce fiind de 4.12 față de -3.84 la începutul primului an; după 2 ani aceste valori au fost de 4.52 -4.74 și, în consecință, amplificând valorile, ceea ce ne indică o stabilitate satisfăcătoare, precum și o osteointegrare bună. Concluzii. În implementarea în practică a protocoalelor de încărcăre funcțională precoce, s-au condus de stabilitatea primară a implantului dentar cu încărcarea precoce a lor după 3-4 săptămâni după implantare, ceea ce a favorizat restabilirea mai rapidă a aspectului estetic și funcțional.
