Morphological changes in the oropharynx in patients with snoring and obstructive sleep apnea syndrome

Abstract

Background. Snoring is a sleep disorder that affects the repose during the night, as a symptom of a serious respiratory condition - obstructive sleep apnea syndrome (SOAS). Objective of the study. Establishing morphological changes in the oropharynx in patients with snoring and OSAS. Material and Methods. Descriptive study. A group of 50 patients with snooring and OSAS were examined oropharyngoscopically. The changes in the uvula, pharyngeal pillars and soft palate were examined. Results. Following the examination, 4 groups with changes of the soft palate and uvula were identified: the first group of 7 patients in which the pharyngeal pillars, the soft palate, the entire uvula are visible; the second group of 18 patients - only the soft palate and the uvula with a slight thickening of the soft palate are visible, especially at the level of the palatoglossal and palatopharyngeal pillars and at the base of the uvula; the third group of 15 patients - visible only the soft palate, the uvula is enlarged in size 2- 3 times; the fourth group of 10 patients - the soft palate is not visible. Conclusion. Patients with chronic rhinopathy and OSAS have changes in the oropharynx that predispose to collapse certain regions of the upper airways. The study showed different groups of patients with hypertrophy of the soft palate that narrows the oropharyngeal anteroposterior surface.

Introducere. Ronhopatia cronică reprezintă o tulburare a somnului care afecteaza odihna pe timpul nopţii ca simptom al unei afecţiuni respiratorii grave – sindromul de apnee obstructivă în somn (SAOS). Scopul lucrării. Stabilirea modificărilor morfologice în orofaringe la pacienții cu ronhopatie cronică și SAOS. Material și Metode. Studiu descriptiv. Un lot de 50 pacienți cu ronhopatie cronică și SOAS au fost examinați orofaringoscopic. Rezultate. În urma examinării, au fost identificate 4 grupuri cu modificări ale palatului moale și uvulei: primul grup de 7 pacienți la care sunt vizibili pilierii faringieni, palatul moale, uvula în întregime ; al doilea grup de 18 pacienți, la care sunt vizibili doar palatul moale şi uvula cu o uşoară îngroşare a palatului moale, în special la nivelul pilierilor palatoglos şi palatofaringian şi la baza uvulei; al treilea grup de 15 pacienți – vizibili doar palatul moale, uvula este mărită în dimensiuni de 2-3 ori; al patrulea grup de 10 pacienți la care nu este vizibil palatul moale. Concluzii. Pacienții cu ronhopatie cronică și SOAS prezintă modificări în orofaringe care predispun la colaps anumite regiuni ale căilor aeriene superioare. Studiul a arătat diferite grupuri de pacienți la care hipertrofia palatului moale îngustează suprafața anteroposterioară orofaringiană.
