Background. The constrained knee prosthesis has the basic indication in revision arthroplasty, but from
the latest literature, result that it has its place in primary total knee arthroplasty in cases of knee
osteoarthritis associated with major deformities. Objective of the study. Evaluation of the method of
surgical treatment with the use of the constrained prosthesis in the difficult primary total knee
arthroplasty. Material and Methods. Our study is based on the surgical treatment, using the constrained
knee prosthesis in the primary total knee arthroplasty, during 2019-2021, of 28 patients with gonarthrosis
associated with severe deformities in varus or valgus, in the Prosthetic Orthopedics of Large Joints
Department, Clinical Hospital of Traumatology and Orthopedics from Chisinau. Results. In our study,
the following criteria were evaluated: the type of deformity - respectively valgus were 10 cases and
varus 18 cases; degree of bone defect - 11 cases were with defects of the joint surfaces larger than 5
mm, where it was necessary to use augmentations; affected side - in 19 cases the right knee was affected
and 9 cases the left one; the F/M ratio was 4/1; the mean age of the patients was 65 years; average
duration of the intervention - 140 minutes; in 5 cases a lateral parapatellar approach was performed, of
which 2 cases with osteotomy of tibial tuberosity; complications - 2 septic cases resolved by
debridement and 1 case with periprosthetic fracture. Conclusion. Osteoarthritis of the knee progresses
rapidly, leading to severe deformities and significant bone defects of the joint surfaces. These cases are
indications for the use of the constrained prosthesis in the primary total knee arthroplasty, having
promising results.
Introducere. Proteza constrânsă de genunchi are indicația de bază în artroplastie de revizie, dar din
datele literaturii de specialitate actuale, rezultă că își are locul și în artroplastia primară în cazurile de
osteoartroza genunchiului asociată cu diformități majore. Scopul lucrării. Evaluarea metodei de
tratament chirurgical cu utilizarea protezei constrânse în artroplastia totală primară dificilă de genunchi.
Material și Metode. Studiul nostru este bazat pe tratamentul chirurgical prin utilizarea protezei
constrânse în artroplastia totală primară de genunchi pe parcursul anilor 2019-2021, a 28 de pacienți cu
gonartroză asociată cu diformități pronunțate în varus sau valgus, în cadrul secției Ortopedia Protetică a
Articulațiilor Mari, IMSP Spitalul Clinic de Traumatologie și Ortopedie din Chișinău. Rezultate. În
studiul nostru s-au evaluat următorare criterii: tipul diformității – respectiv valgus au fost 10 cazuri și
varus 18 cazuri; gradul defectului osos – 11 cazuri au fost cu defecte a suprafețelor articulare mai mari
de 5 mm, unde au fost necesare utilizarea augumentelor; partea afectată – în 19 cazuri a fost afectat
genunchiul drept și 9 cazuri cel stâng; raportul F/B a fost de 4/1; vârsta medie a pacienților a fost de 65
de ani; durata medie a interveției – 140 minute; în 5 cazuri s-a efectuat abord parapatelar lateral dintre
care 2 cazuri cu osteotomia tuberozitații tibiale; complicații – 2 cazuri septice rezolvat prin debridare și
1 caz cu fractura periprotetică. Concluzii. Osteoartroza de genunchi progresează rapid, ducând la
diformități severe și defecte osoase importante a suprafețelor articulare. Aceste cazuri sunt indicații către
utilizarea protezei constrânse în artroplastia primară totală de genunchi, având rezultate promițătoare.
