Arthroscopic ACL reconstruction

Abstract

Background. Currently the methods of arthroscopic stabilization of LIA are widely used in sports orthopedics. The literature describes the advantages and disadvantages of using different grafts. Objective of the study. Rate the postoperative results of the reconstruction of the anterior cruciate ligament (ACL) under arthroscopic control with the use of autograft from the tendon of semitendinosus and gracilis muscles with the use of the lock,“endobutton”. Material and Methods. The analysis of the treatment results of 92 patients with ACL injury of the knee, and the anterior-medial decompensated instability in the period from 2018 to 2021,was conducted. The representation of the group included 72 men and 20 women, with the average age of 26 years old (range 18 to 45 years). Results. Postoperative, the patients were followed up for 6-48 months, using the LYSHOLM scale. Based on these studies, excellent and good results were obtained in 83 patients (90%), satisfactory in 9. All athlete patients resumed training after 6 months. Conclusion. 1. In most cases, the ACL injury is the result of sports injuries: football, skiing, contact sports. 2. ACL autoplasty with transplantation of the gracillis and semimembranosus tendon provides an anatomical reproduction of the ligament.

Introducere. La momentul actual metodele de stabilizare artroscopică a LIA sunt utilizate pe scară largă în ortopedia sportivă. Literatura de specialitate descrie avantajele și dezavantajele utilizării diferitelor grefe. Scopul lucrării. Evaluarea rezultatelor postoperatorii ale reconstrucției ligamentului încrucișat anterior (LIA) sub control artroscopic cu utilizarea autogrefei din tendonul mușchilor semitendinos și gracilis cu utilizarea încuietorii, „endobuton”. Material și Metode. A fost efectuată analiza rezultatelor tratamentului a 92 de pacienți cu leziuni LIA ale genunchiului și a instabilității decompensate anteriormediale în perioada 2018-2021. Reprezentarea grupului a inclus 72 de bărbați și 20 de femei, cu vârsta medie de 26 de ani (intervalul 18-45 de ani). Rezultate. Postoperator, pacienții au fost urmăriți timp de 6-48 de luni, utilizând scala LYSHOLM. Pe baza acestor studii s-au obținut rezultate excelente și bune la 83 de pacienți (90%), satisfăcătoare – la 9. Toți pacienții sportivi au reluat antrenamentul după 6 luni. Concluzii. 1.În majoritatea cazurilor, leziunea ligamentului încrucișat anterior este rezultatul traumatismelor sportive: fotbal, schi, sporturile de contact. 2. Autoplastia LIA cu transplant din tendonul gracillis și semimembranos oferă o reproducere anatomică a ligamentului.
