Coronectomy of the wisdom teeth: the dental extraction alternative

Abstract

Background. Coronectomy of the wisdom teeth is an alternative surgical procedure to tooth extraction, which aims to preserve the roots of the wisdom teeth in the dental socket after surgical separation of the crown, in order to avoid the injury of the inferior alveolar vascular-nervous bundle. Objective of the study. Presentation of a clinical case of coronectomy and analysis of the local and general postoperative status of the patient in relation to the data evoked in the literature based on the respective subject. Material and Methods. A 28 years old patient was examined clinical and paraclinical (CBCT was performed), the intensity of pain was assessed according to SAV (from 1 to 10), the status of the soft tissue was stated on day 3, 7 and 14 via IVP (from 1 to 3). Results. At the postoperative evaluation of the patient according to the SAV at 3 days postoperatively, the following values were obtained: 5 on day 3, 3 on day 5, 1 on day 14. The clinical examination of the postoperative wounds according to IVP showed the following data: 3rd day- 2, 7th day- 1, 2 weeks- 1. Conclusion. Both the subjective and the objective evaluation method used in the study confirmed the existing literature data that describe the local status of the postoperative wound, coronectomy being an efficient method of treatment for avoiding the IAN injury.

Introducere. Coronectomia molarilor de minte este un procedeu chirurgical alternativ extracției dentare, ce urmărește păstrarea rădăcinilor dinților de minte în alveolă, după separarea chirurgicală a coroanei, cu scop de a evita lezarea fasciculului vasculo-nervos alveolar inferior. Scopul lucrării. Prezentarea unui caz clinic de coronectomie și analiza statutului local și general postoperator al pacientului, în raport cu datele evocate în literatura de specialitate la subiectul respectiv. Material și Metode. Pacientul cu vârsta de 28 de ani a fost examinat clinic și paraclinic (CBCT), postoperator, iar la 3,7 și 14 zile s-a evaluat intensitatea durerii prin intermediul SAV (de la 1 la 10) și starea țesuturilor moi la 3,7 și 14 zile, prin intermediul IVP (de la 1 la 3). Rezultate. La evaluarea postoperatorie a pacientului, conform SAV la 3 zile postoperator, s-a obținut valoarea 5, la 7 zile- valoarea 3, la 14 zile- 1. La examenul clinic obiectiv al plăgii postoperatorii conform IVP la 3 zile a fost obținută valoarea 2, la 7 zile- 1, la 14 zile- 1. Concluzii. Atât metoda de evaluare subiectivă, cât și cea obiectivă utilizată în studiu au confirmat datele literaturii de specialitate, în raport cu statutul local a plăgii postoperatorii, coronectomia fiind o metodă fiabilă, ce permite evitarea lezării nervului alveolar inferior.
