Метод миорелаксации жевательных мышц как основной фактор предваряющий определение центральной окклюзии в лечении больных МСД ВНЧС окклюзионными лечебными шинами

Abstract

Резюме. Исследование посвящено изучению эффективности лечения больных МСД ВНЧС окклюзионными лечебными шинами, изготовление которых проводится на основе определения центральной окклюзии. Известно, что жевательные мышцы у больных МСД ВНЧС находятся в разбалансированном состоянии, что приводит к «вынужденному» положению нижней челюсти. Для определения истинной центральной окклюзии, предложены различные методы предварительной релаксации жевательных мышц. Под наблюдением находилось 50 больных, которых разделили на 5 групп, по 10 пациентов в каждой. Перед определением (ц.о.) миорелаксацию в 4–х исследуемых группах проводили на основе 4–х общеизвестных методов. В контрольной группе миорелаксацию не проводили. После определения (ц.о.) пациентам всех групп, включая контр ольную, были изготовлены окклюзионные лечебные шины. Исследования в течение 3–х месяцев показали, что сроки и эффективность лечения окклюзионными шинами очень различны и зависят от способа релаксации жевательных мышц. По всем показателям (сроки устранения симптомов заболевания, количество коррекций окклюзионной поверхности, результаты ЭМГ жевательных мышц, АФГ, КТ, УЗИ суставов) самые высокие результаты выявлены в 5–й группе больных, миорелаксацию которым проводили по методу, предложенному Бояном А.М. с соавторами (патент №49503 от 26.04.2010). Самые низкие результаты были в группе №1, больным которой, предварительная миорелаксация перед определением ц.о. не проводилась.

Rezumat. Studiul este consacrat examinării eficienței tratamentului disfuncției ATM prin aplicarea capelor ocluzale confecționate în temeiul rezultatelor aprecierii ocluziei centrale. Se cunoaște că, mușchii masticatori la pacienții cu disfuncție ATM se află în stare dezechilibrată, fapt care determină o poziție habituală a mandibulei. Pentru aprecierea ocluziei centrale veridice sunt propuse diferite metode de relaxare a mușchilor masticatori. Studiul s-a efectuat pe un eșantion de 50 pacienți divizați în 5 grupe a cîte 10 pacienți. La 4 grupe s-a efectuat miorelaxarea prin 4 metode bine — cunoscute, iar în grupa de control miorelaxarea n-a fost efectuată. Ulterior aprecierii ocluziei centrale, tuturor pacienților, inclusiv din grupa de control, li s-au confecționat cape ocluzale. În rezultatul monitorizării pacienților pe parcursul a 3 luni s-a constatat că durata și eficiența tratamentului prin intermediul capelor ocluzale diferă și depind de metoda relaxării mușchilor masticatori. Analizînd toți indicatorii (termenul dispariției simptomelor bolii, numărul corecțiilor suprafeței ocluzale, rezultatele EMG mușchilor masticatori, AFG, TC, USG articulațiilor) s-a conchis că cele mai înalte rezultate au fost obținute în grupa de pacienți cărora li s-a efectuat miorelaxarea conform metodei propuse de Boian Arcadie și coautorii (patenta nr. 49503 din 26.04.2010), iar cele mai joase — în grupa de pacienți cărora miorelaxarea n-a fost efectuată.

Summary. The study investigates the efficacy of treatment TMD patients by occlusal therapeutic splints, production of which is based on the registration of central occlusion. It is known that masseter muscles in TMD patients are in unbalanced state resulting a “forced” position of the mandible. In order to register the true central occlusion various methods of pre-relaxation of the masseter muscles are proposed. We observed 50 patients who were divided in 5 groups of 10 patients which. Masseter muscles relaxation in the 4 studying groups were performed on the basis of 4 well-known methods. In the control group muscle relaxation was not performed. After registration of central occlusion the patients of all groups were provided with occlusal therapeutic splints including the control group. The researches held within 3 months have shown that the time and the effectiveness of the treatment by occlusal therapeutic splints are very different and depend on the method of relaxation of the masseter muscles. According all medical parameters (terms of elimination of disease symptoms, the number of corrections of the occlusal surface, EMG of masseter muscles, arthrophonography (AFG), CT, ultrasound of the joints) the highest results were identified in the group of patients No.5 whom muscle relaxation were conducted on the method proposed by Boyan A.M. et al. (Patent No. 49503 from 26.04.2010). The lowest results were in the group No.1 where masseter muscle relaxation was not carried out.
