Introducere. Gingivita hiperplazică reprezintă un proces proliferativ, histopatologic caracterizat prin multiplicarea celulelor din straturile bazal şi spinos. Administrarea zilnică şi permanentă a grupelor de preparate (antiepileptice, imu-nosupresoare, hipotensive) poate provoca reacţii adverse ca hiperplazia gingivală. Scop. Analiza aspectelor etiologice şi clinico-morfologice caracteristice gingivitei hiperplazice de etiologie medicamentoasă în literatura pe specialitate naţională şi de peste hotare. Material şi metode. Studiul a inclus analiza a 374 surse bibliografice din PubMed, Cohrane Library, Reasearch Gate cu gingivită hiperplazică de etiologie medicamentoasă. Administrarea Dilantinei a raportat în 50% din cazuri de apariţie a hiperplaziei gingivale, Nifedipina în 25% cazuri, Ciclosporina după 35 de ani variază între 8-70 % cazuri clinice. Rezultate. Examenul histopatologic confirmă diagnosticul clinic şi determină în ţesutul gingival: hiperkeratoză, parakeratoză, acantoză în epiteliu; creşterea numărului de fibroblaşti şi vase de neo-formaţie cu apariţia unui ţesut conjunctiv tânăr; fenomene distrofice epiteliale; vase sanguine dilatate cu aspect tel-eangiectatic; bogat infiltrat inflamator limfo-plasmocitar în corionul gingival. Proliferarea gingiei, drept răspuns la o anumită medicaţie sistemică, însoţită de administrarea de lungă durată, susceptibilitatea la preparate, deseori asociată cu deficienţa igienei bucale, solicită metoda chirurgicală în restaurarea conturului gingival. Concluzii. Gingivita hiperplazică de etiologie medicamentoasă reprezintă un capitol aparte, care necesită un studiat detaliat. Menţinerea igienei orale riguroase, monitorizarea afecţiunii sistemice şi înlocuirea preparatelor farmacologice au un rol important în profilaxia şi tratamentul afecţiunii.
Introduction. Hyperplastic gingivitis is a proliferative, his-topathological process characterized by the multiplication of cells in the basal and spinous layers. Daily and permanent administration of groups of drugs (antiepileptic, immunosuppressive, hypotensive) can cause adverse reactions such as gingival hyperplasia. Objective. Analysis of the etiological and clinical-morphological aspects characteristic for drug-induced hyperplastic gingivitis in the national and foreign specialized literature. Material and methods. The study included the analysis of 374 bibliographic sources from PubMed, Cochrane Library, Research Gate with drug-induced hyperplastic gingivitis. Dilantin administration was reported in 50% of cases of gingival hyperplasia, Nifedipine in 25% of cases, and Cyclosporine after 35 years varies between 8-70% of clinical cases. Results. The histo-pathology confirms the clinical diagnosis and determines: hyperkeratosis, parakeratosis, acanthosis in the epithelium; increased number of fibroblasts and neoformation vessels with the appearance of young connective tissue; epithelial dystrophic phenomena; dilated blood vessels with a telangiectatic appearance; rich inflammatory lympho-plasmocytic infiltrate in the gingival chorion. Proliferation of the gingiva as a response to a certain systemic medication, accompanied by long-term administration, susceptibility to preparations, often associated with poor oral hygiene, requires the surgical method in restoring the gingival contour. Conclusion. Drug-induced hyperplastic gingivitis represents a separate chapter, which requires detailed study. Maintaining rigorous oral hygiene, monitoring the systemic condition and replacing pharmacological preparations have an important role in the prophylaxis and treatment of the pathology.
